PLEASE READ THE ENTIRE SOLICITATION PACKAGE.

In order to submit an offer, complete and return an original proposal plus two (2) copies of the information as shown in subject solicitation, Sections L & M, to the 4th Contracting Squadron/LGCA/B Team, 1695 Wright Brothers Avenue, Seymour Johnson AFB NC 27531-2459. Example of requested information (not all inclusive) include:

Standard Form (SF) 1442

Solicitation, Section B – Supplies or Services and Prices Solicitation, Section K, Representations, Certifications and Other Statements

All Requirements of Solicitation, Section L, Instructions, Conditions and Notices

Determination of Contractor Responsibility Information NOTE: The 4th Contracting Squadron will not maintain information provided in an offer for this solicitation for use on offers provided for other solicitations. Each solicitation/offer is stand-alone and offerors shall provide complete and independent offers for each solicitation.

NOTE: Section K, Representations and Certifications sets forth specific information on eligibility requirements for this acquisition. Ensure Contractor Online Representations & Certifications Application is complete and updated in addition to Section K, Representations and Certifications in the solicitation.

NOTE: Section L (Instructions, Conditions and Notices) and Section M (Evaluation Factors for Award) require special attention and provide further guidance on the contents of an offer.

NOTE: All amendments must be acknowledged by completing Block 19 on the reverse of the SF 1442 or by submitting the SF 30, with Blocks 15 A and B completed, prior to the date and time offers are due.

NOTE: Ensure On-line Representations and Certifications Application (ORCA) has been completed and/or updated at http://www.bpn.gov/orca .

**FAILURE TO PROVIDE ANY OF THE ABOVE MAY RESULT IN AN OFFER BEING DETERMINED NON-RESPONSIVE AND INELIGIBLE FOR AWARD.**

Please note Section G PAYMENT: The AWARDEE OF THE CONTRACT RESULTING FROM THIS RFP Shall submit payment requests using Wide Area WorkFlow-Receipt and Acceptance (WAWF-RA).

Questions concerning this acquisition may be addressed prior to the Site Visit to the individual named in Block 9A on the SF 1442, Solicitation, Offer and Award.

The only site visit that will be held is scheduled for 10:00 am on 20 Jan 2011 at the 4th Contracting Squadron. Submit names of all attendees (not to exceed two per contractor) to TSgt Latesha Flowers, by e-mail to latesha.flowers@seymourjohnson.af.mil or by fax prior to 18 Jan 11 at 12:00 PM.

Use the attached Checklist when preparing an offer.

OFFEROR CHECKLIST

PLEASE CHECK

a. Have amendments to the solicitation been acknowledged?

b. Have all requested prices been inserted in the applicable blanks on Section B – Supplies or Services and Prices?

c. Have all prices and computations been checked to preclude mathematical mistakes, misplacement of decimal points and transposition of numbers?

d. Is the offer submitted on the latest Section B – Supplies or Services and Prices? (If applicable, Technical Proposal/Sample Project pricing-AF Form 3052 and Offer Schedule, included as attachment to the Solicitation)

e. Have all proposed prices/cost been inserted appropriately on the front and reverse of the AF Form 3052 (Construction Cost Breakdown), and submitted with the Offer (if requested)?

f. Is the name and title of person authorized to sign entered in Block 20 A of the SF 1442?

g. Is the offer dated and signed in Blocks 20 B and C of the SF 1442?

h. Is the offer signed by a person legally authorized to bind the offeror and identified in Clause 52.203-2, in the contractor's On Line Representations & Certifications Application (ORCA)? http://www.bpn.gov/orca

i. Is the offer signed in accordance with Federal Acquisition Regulation (FAR) 4.102?

j. Have all appropriate boxes been checked in Section K, Representations, Certifications and Other Statements; and is that Section submitted with the offer? Is ORCA updated, and all appropriate blocks marked?

k. Is the CAGE Code (Commercial and Government Entity) number included in Item 14, SF 1442? Is the Tax ID Number included? Is the DUNS Number included?

l. Is the offeror registered in the Central Contractor Registration (CCR) database? http://www.ccr.gov/ Is the CCR record current/updated to include pertinent information (i.e., NAICS/size standards, etc)?

m. Has the offeror completed/updated electronic annual representations and certifications at http://www.bpn.gov/orca ?

CAUTION: Offers that are qualified by the offeror or incomplete may be considered NON-RESPONSIVE.
