MEMORANDUM FOR: MATOC CONTRACTORS

SUBJECT: Fair Opportunity Proposal Request (FOPR) under the MATOC: Project OPP1071525, Construct RV Sewage Holding Tank; Clear Lake
1. This is a FOPR for subject project to be awarded on a fair opportunity basis pursuant to Federal Acquisition Regulation (FAR) 16.505 ordering procedures, the terms and conditions of the basic MATOC solicitation, and the specific requirements outlined in this request with attachments.
2. TASK ORDER DESCRIPTION: Fairchild AFB invites you to submit a proposal for Project OPP1071525, Construct RV Sewage Holding Tank, Clear Lake. This is a Design-Build project, defined as a specific Work Element No. 1.2 proposed task order as specified in the basic MATOC contract requirements.
a. Work includes, but is not limited to: design and construction to install sewage holding tanks, concrete pad for the tanks, sewage lift station, associated sewer and electrical lines, and screen fencing at Fairchild AFB’s Clear Lake Recreation Center. The requirements of the attached Statement of Work shall also be met.
3. PROJECT MAGNITUDE: The project magnitude is between $100,000 and $250,000.
4. PERFORMANCE PERIOD: Total Performance Period for this project is 330 calendar days and will be performed in three distinct phases, as applicable and defined below:
Phase I - Phase I is identified as the Design, Material Gathering/Submittal Phase, and has a duration of 75 calendar days to prepare, submit, and receive approval on all submittals, other than those required for closeout.
Phase II - Phase II is identified as the Physical Work Phase and has a duration of 180 calendar days to occupy the work site and to physically accomplish the work.
Phase III - Phase III is identified as the Closeout Phase and has a duration of 75 calendar days for completion of all closeout requirements specified.
5. NOTICE TO PROCEED: A Notice to Proceed (NTP) be issued for each phase of work. Request NTP for Phase I be issued within 15 calendar days after award of the task order and upon receipt and approval of bonds, the contractor’s Quality Control Plan. Request NTP for Phase II/III be issued upon substantial completion of Phase I/II work, respectively. No NTP shall be issued until the previous phase has been formally accepted and/or recommended for issuance by CENM.
6. WINTER EXCLUSION: Winter exclusion period, pertinent to the physical work phase, is not requested.

7. LIQUIDATED DAMAGES: Liquidated damages shall apply for all phases- Design, Material Gathering/Submittal; Physical Work; and Closeout. Breakdowns for each phase are as follows:
• LDs for Phase I will be assessed in the amount of $183.00 per day.
• LDs for Phase II will be assessed in the amount of $228.00 per day.
• LDs for Phase III will be assessed in the amount of $158.00 per day.
8. WORKING HOURS: Working hours are weekdays, 7:30 AM to 4:30 PM, excluding federal holidays.
9. SUPERINTENDENT/ QC SUPERINTENDENT: The Superintendent/QC responsibilities may be combined. QC requirements of the basic contract shall be met; Contractor Quality Control shall also conform to UFGS, Guide Specification 01 45 00.00 10 (revision Nov 2016).
10. QUALITY CONTROL PLAN: Within 15 calendar days after the award of the task order, the Contractor shall submit an Interim Quality Control Plan per the provisions of the UFGS Guide Specifications, Section 3.2.
11. WAGE DETERMINATION: The most current “Building” wage determination for Spokane County referenced in attachment 8A of the solicitation is applicable to this project.
12. BONDS: The successful offeror will be required to submit a performance and payment bond in the amount of 100% of the task order price within fifteen (15) calendar days after task order award. A bid bond is not required.
13. RECOVERED MATERIALS: Recovered Materials Determination and Green Procurement are applicable for the project. Anticipated recovered materials for project use include: cement and concrete containing fly ash or ground granulated blast furnace slag and non-pressure pipe.
14. CONFLICT OF INTEREST: Identify any conflict of interest or appearance issues which may be a potential Organizational Conflict of Interest (OCI) in accordance with FAR 9.5, as supplemented, and any basic contract clause(s) that govern the IDIQ. OCI mitigation plans or a statement that no OCI exists should accompany your proposal. Contractors are responsible for immediately notifying the Contracting Officer should any potential conflict of interest arise at any time prior to and/or after contract award.
15. PRE-PROPOSAL SITE VISIT: A pre-proposal site visit will be held on 14 May 2019 9:00 A.M.
(local). Parties will meet at the Contracting Office, Bldg. 2451A Fairchild AFB, WA. Attendees must be pre-cleared to access the base. All interested contractors should contact David Kovalchuk, (509) 247-4869, david.kovalchuk@us.af.mil at least 3 days prior to the site visit to request clearance for access to the base.
16. OFFERS DUE: Proposals shall be submitted to this office no later than 31 May 2019 at 2:00 P.M.
(local), unless changed by Amendment. Proposals submitted after the established submission time will be “late” in accordance with FAR 52.215-1(c)(3). Submit proposals via email to david.kovalchuk@us.af.mil, AND paul.blais@us.af.mil or by fax to (509) 247-8685.
17. PRICE SCHEDULE: Proposals shall be submitted to this office in accordance with Section L of the solicitation and evaluated for award in accordance with Section M of the solicitation. Price proposal shall consist of completing the Government provided price schedule (Attachment 1). Proposals shall indicate that offers are valid until 30 September 2019 for Government acceptance, as well as acknowledgements of all amendments issued to the FOPR.
18. QUESTIONS AND ANSWERS: Any questions, whether technical or contractual in nature, shall be submitted in writing. Questions and Answers may be submitted in writing to the Contract Specialist
