About this file

This is a Past Performance Questionnaire template issued by the 87th Contracting Squadron for evaluating contractor performance on previous federal contracts.

The questionnaire is designed to be completed by government representatives or project managers who have direct knowledge of a contractor's performance on a specific contract. It captures information about both the offeror (the contractor being evaluated) and the respondent (the government representative providing the assessment). The form requires detailed contract information including contract title, number, contracting officer contact details, contract amount, type, performance period, and place of performance. Respondents evaluate contractor performance across three primary categories using a 1-5 scale: Performance and Supportability (five criteria including service quality, personnel qualifications, deliverable completion, labor turnover, and responsiveness to issues); Management (five criteria covering overall management effectiveness, communication with the COR/PM, cooperation with government, subcontractor management, and quality control); and Timeliness/Schedule (five criteria addressing personnel recruitment, retention, replacement planning, credentialing package timeliness, and complaint resolution). The scale definitions range from Exceptional (5) indicating performance exceeding requirements with very low risk, through Satisfactory (4) with moderate risk, to Unsatisfactory (2) with very high risk, with a Neutral (1) option for contractors with minimal relevant past performance. The questionnaire also includes yes/no questions regarding contract award likelihood, default termination, corrective notices, and requires an overall performance rating with mandatory supporting comments for any Unsatisfactory, Marginal, or Exceptional ratings. Completed questionnaires must be returned via email to SSgt Alexander Kouba and Mr. Rodney Brown at the provided addresses.
