About this file

This document is a Past Performance Questionnaire template issued by the 87th Contracting Squadron for use in evaluating contractor performance on federal contracts.

The questionnaire is designed to be completed by contracting officers or government representatives who have direct knowledge of a contractor's performance on previous contracts. It collects structured information across three main sections: Offeror's Information, Respondent Information (the entity completing the questionnaire), and Contract Information (details about the specific contract being evaluated). Respondents rate contractor performance on a standardized 1-5 scale with corresponding risk levels: Exceptional (5, Very Low Risk), Satisfactory (4, Moderate Risk), Marginal (3, High Risk), Unsatisfactory (2, Very High Risk), and Neutral (1, Unknown Risk). The evaluation criteria are organized into three categories: Performance and Supportability (five items assessing service quality, personnel qualifications, deliverables, labor stability, and issue responsiveness); Management (five items covering management effectiveness, communication, customer cooperation, subcontractor management, and quality control); and Timeliness/Schedule (five items addressing recruitment capability, personnel retention, replacement planning, credentialing timelines, and complaint resolution). Section F requires yes/no responses regarding contract award recommendations, default termination, cure notices, and an overall performance rating. All Unsatisfactory, Marginal, or Exceptional ratings require supporting comments; failure to provide comments renders the questionnaire void and unacceptable to the government. Completed questionnaires must be returned via email to alexander.kouba@us.af.mil.
