MEMORANDUM FOR RECORD
SUBJECT: Site Visit, FA441720R0009 Water Tower Maintenance Indefinite-Delivery Indefinite-Quantity (IDIQ), Hurlburt Field, FL.
1. A site visit was accomplished 09 July 2020 at 01:00 PM, Local Time.
2. Everyone was asked to sign in and to check the appropriate block if they would like to have their names posted on beta SAM website for subcontractors and suppliers.
3. Introductions were made: Contracting Officer – SSgt Lukas Anderson; Contract Specialist – Melissa Mattis; Government Inspectors – Mr. John Foster, Mr. Jeremy Isaac, and Mr. Alan Cox.

4. The following items were discussed:
a. Solicitation number FA441720R0009 with a current closing date of 30 July 2020 at 2 PM Local Time.
b. Potential offerors are encouraged to subscribe for real-time e-mail notifications when information has been posted to the website https://beta.sam.gov for this solicitation.
c. HARD COPIES OF PROPOSALS WILL NOT BE ACCEPTED except for hard copy bid bonds. Bid bonds may be either a hand carried hard copy, a hard copy sent by mail by the time specified in this solicitation, or, in response to the Coronavirus Disease 2019 (COVID-19), may alternatively be submitted electronically with “electronic signatures and electronic, mechanically-applied, or printed dates” in accordance with and approval referenced by Class Deviation 2020-O0016, Original Documents, Signatures, Seals, and Notarization dated 30 April 2020. Only electronic versions of the proposals will be accepted.
Paper copies of proposals will not be evaluated, will be rejected, and will not be considered for award. Proposals may be submitted electronically in one of the two ways identified in Section L of the solicitation.
d. The time clock outside of the large conference room will be used to determine the time proposals are received… And one minute late is still late.

e. Base Entry: Personnel requiring access to Hurlburt Field for the purpose of delivering an electronic proposal on compact disk (CD) must send an e-mail containing the company name and the name of the person(s) requiring access to the following individual: SSgt Lukas Anderson, email lukas.anderson@us.af.mil. Requests must be received no later than (NLT) 24-hours prior to the date and time established for receipt of proposals. All commercial vehicles will need to use the new Commercial Vehicle Inspection (CVI) gate located off of Martin Luther King Jr Blvd. Personal Owned Vehicles will use the main gate off of Hwy 98.
Please allow extra time to obtain your pass.
f. Please review solicitation documents carefully especially Sections L and M of the solicitation to ensure compliance with all proposal submission requirements. Section L and M contain the proposal preparation instructions and evaluation factors for award. Failure to ensure compliance with all submission requirements could potentially make an offer non-responsive and ineligible for award consideration.
g. A complete proposal shall consist of the following FOUR (4) sections:
(a) Volume I – Technical Proposal Subfactor 1 – SEED Project Schedule Subfactor 2 – Management Plan and Qualifications
(b) Volume II (a) – Past Performance
(c) Volume II (b) – Signed Teaming Agreement or Joint Ventures (required if
Teaming Agreement or Joint Venture is proposed)
(d) Volume III – Cost/Price Proposal and Contractor Responsibility
(e) Volume IV – SEED Project Proposal
h. In regards to Past Performance, it is your responsibility to mail out past performance questionnaires to your previous customer contacts, and ensure they are completed and returned to the contracting point of contact (POC) no later than the closing date of the solicitation.
i. In lieu of a Past/Present Performance Questionnaire, if a project is currently available in the Contractor Performance Assessment Reporting System (CPARS) the contractor should attach the evaluation to the questionnaire with the relevant solicitation number and submit directly to the 1st Special Operations Contracting Squadron office before the official closing date and time.
j. Attendees are advised of the requirement to submit all questions in writing to the Contracting office NLT close of business (COB) five (5) business days following the site visit, which is currently 16 July 2020. All questions must be submitted in Word format. Any questions asked during the site visit may not be answered at a later time in writing.
