PAST AND PRESENT PERFORMANCE QUESTIONNAIRE

SUBJECT: Request for Past Performance Evaluation

TO:

You have been identified as a point of contact for a past and/or present performance evaluation of the firm listed on the attached questionnaire. This firm is currently being considered for a Firm Fixed Price contract at Sheppard AFB, TX. Any comments you can provide regarding the past performance of the contractor providing this form is greatly appreciated.

Please complete the attached questionnaire for the contractor and program indicated. Please fill in all sections. State "not applicable" if the question does not apply. Send the completed questionnaire to Christine Docker, Contract Administrator, christine.docker.2@us.af.mil or Cecilia P. Murray, cecilia.murray@us.af.mil. Please title the subject line of e-mail, "FA3020- 17-R-0002 – PP Questionnaire". Once completed, the questionnaire becomes a Source Selection Sensitive document and must be handled in accordance with FAR 2.101 and FAR

3.104. PLEASE DO NOT SEND THE COMPLETED QUESTIONNAIRE TO THE

CONTRACTOR.

S Satisfactory

Performance meets contractual requirements. The contractual performance of the element being assessed contained some minor problems for which corrective actions taken by the contractor appear or was satisfactory.

M Marginal

Performance does not meet some contractual requirements. The contractual performance of the element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s actions appear only marginally effective or were not fully implemented.

U Unsatisfactory

Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element being assessed contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

N Not Applicable Unable to provide a score. Performance in this area is not applicable to the effort assessed.
