WASHER AND DRYER LEASE AND MAINTENANCE SERVICES

FORT SAM HOUSTON & CAMP BULLIS, TX

1. DESCRIPTION OF SERVICES/GENERAL INFORMATION. The contractor shall provide all personnel, transportation including fuel, tools, supplies, supervision, equipment and other items and services necessary for the

Washer/Dryer Maintenance, Repair and Leasing Service Contract as defined in this Performance Work Statement

(PWS) at Joint Base San Antonio, Fort Sam Houston. The contractor shall be responsible for the delivery, installation, cleaning, maintenance, repair, removal, and relocation of contractor furnished washers and dryers.

1.1. DESCRIPTION OF SERVICES

1.1.1. EQUIPMENT QUALITY STANDARDS. All washers and dryers shall be in compliance with Executive

Order 12902 (Sec. 507) and Energy Policy Act of 1992 (EPACT). All washers shall be efficient and comply with the Energy Star ratings required. See website at http://www.energystar.gov for brands and model numbers that meet the U.S Department of Energy qualifications. All machines shall be new at time of initial installation. Replacement appliances shall not be older than the current age of the machine installed at the beginning of the contract. New or replacement equipment shall be the same size and color.

1.1.2. LEASE APPLIANCE. The government, at a monthly rate per appliance, shall lease appliances. Lease occurring during the month of delivery of any appliance and the month of return of any appliance to the contractor shall be prorated based on a 30-day month. Appliances that are removed and not replaced to operating condition under the terms of paragraph 1.3, Maintenance Services, shall be deducted from the lease fee at a rate of 1/30 of the monthly leasing fee of the removed appliance per day. Deductions shall not be made if appliances are replaced or restored to operation condition starting from the day after the allowed time for maintenance/service.

1.1.3. INSTALLATION. The contractor shall provide the quantity of washers and dryers, required by this PWS, Appendix A, completely installed, leveled, and ready for use. The appliances shall be heavy duty, commercial or industrial brand and designed to operate continuously at its maximum design capacity/efficiency. Contractor shall be responsible for all appliances provided. The government assumes no responsibility for any damage or loss except as provided in paragraph 1.3.5.

1.1.4. INSTALLATION SCHEDULE. The contractor shall submit an installation schedule for all appliances for approval at the pre-performance conference. The schedule shall include delivery and installation of all appliances in designated buildings. See Technical Exhibit 1.

1.1.5 PRE & POST AWARD CONFERENCE/PERIODIC PROGRESS MEETINGS. The Contractor agrees to attend any pre and post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The CO, COR, and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.1.6. APPLIANCE CONNECTIONS. Connections of appliances to existing government-furnished utilities shall be coordinated with 502 CES/CEOSS, BLDG 4197. Final work shall be subject to the approval of the Contracting

Officer’s Representative (COR) and shall be installed in accordance with manufacturer's recommendations. The contractor shall ensure that installation is compatible with existing facilities. Only the contractor, with proper coordination through the CO and COR, shall remove or relocate appliances for any reason.

1.1.7. WASHERS. The contractor shall furnish new washing machines as indicated in Appendix A. Washers shall be heavy duty commercial or industrial brand and design and have the minimum amount of control knobs, switches, settings and cycles. The washers shall be white in color. Washers shall be equipped with "push-to-start" switches or buttons, which only operate when door is closed. Heavy-duty machines, not designed specifically for commercial use, shall not be acceptable. The contractor shall provide catalogs, brochures, or other written literature and specifications on washer equipment the contractor proposes to furnish under this contract for approval. Literature shall be provided with submission of offer proposal. Contractor shall furnish all parts and accessories and make all connections (hot and cold-water hoses, drain hose, etc.) to make equipment fully operational. The hot and cold water lines and drain hoses are not cost reimbursable. Washers shall be equipped with a UL listed, molded three (3) prong 110 V male plug.

1.1.8. DRYERS. The contractor shall furnish new electric and gas dryers as indicated in Appendix A. Dryers shall be heavy-duty machines designed specifically for commercial use and shall be of a matched color with washing machines (white). All dryers shall be equipped with manual "push-to-start" switch and electronic ignite which only operate when door is closed. If dryers are equipped with coin buttons or switches, the coin slot shall be disabled;

blocked or plugged. All machines shall have the capability of being manually turned off irrespective of the time into drying cycle. Dryers shall have variable heat settings. The dryers shall be of 18-pound capacity single stack, (Gas-ll0V, E1ectric-220V). The contactor shall furnish all flexible gas lines for gas dryers. The electrical cords are not cost reimbursable. Dryers shall have door mounted filters. All dryers must be installed with flexible aluminum vents, which will be non-reimbursable.

1.1.9. CONTROLS AND CONTROL KNOBS. All controls and control knobs shall be the sole responsibility of the contractor.

1.1.10. APPLIANCE IDENTIFICATION CARDS. All appliances provided shall be identified as property of the contract. Appliance "Operating Instructions" decals shall be prominently displayed in view of customers.

Contractor shall furnish the appliance identification/signs or any additional signs deemed necessary, to the COR within five (5) days prior to contract start date for approval.

1.1.11. MAINTENANCE/REPAIR SERVICES. The contractor shall be responsible for the machine to include the hook-up to government utilities. The contractor shall monitor, inspect, and clean (appliance surfaces) all washers and dryers monthly to ensure equipment is operational and performing according to design. The contractor shall submit a monthly maintenance and cleaning schedule to the respective COR at least five (5) business days before the start of each month. The schedule shall include the date and approximate time each machine shall be serviced. Preventative maintenance shall be performed to identify potential problems and to repair prior to equipment failure. Washer and dryer connections such as hoses, exhaust vents, gas lines, etc., shall be inspected during the scheduled equipment inspections. The contractor shall inspect and clean all dryer exhaust vents monthly, from the dryer itself to the permanent exhaust system. In facilities where multi dryer ducts are available, the contractor will be allowed to shut off only one system at a time for cleaning. The contractor shall inspect dryer lint filters and immediately replace torn or unusable filters. Contractor shall ensure dryer exhaust vents are properly secured to dryer and exhaust ducting at all times to prevent fire hazards. Contractor shall identify and replace deteriorated ducting as part of the monthly cleaning schedule. Contractor shall ensure areas surrounding dryers are free from lint, debris, etc, after each service.

1.1.12. SERVICE CALL. Service repair calls will occur when appliances malfunction or fail to operate properly.

The contractor shall pick up Service Request tickets from the COR, Monday thru Friday, no later than 8:00 A.M.

The contractor shall respond to all service repair calls as requested by the respective COR within 48 hours.

Contractor shall place an “OUT OF ORDER” sign on inoperable machines with an expected date of repair for unserviceable equipment. The contractor shall also check for other problems to washers/dryers in the area. Upon completion of the service call, the contractor shall identify the response date, provide a short summary of repair performed or reason equipment was not made operational to the respective COR. The contractor shall report to 502

CES/CEOSS, Bldg 4197 prior to responding to a service call.

1.1.13. REPLACEMENT. Any appliance that cannot be repaired to operating condition on site within three (3) working days shall be identified to the COR and shall be removed and replaced with new appliances that are the same type and brand. The contractor shall maintain sufficient replacement appliances, ready for use, in the event major repairs require removal of any appliances.

1.1.14. FREQUENT REPAIRS. Appliances that show a continued tendency for frequent repairs as determined by the COR shall be removed and replaced with a new appliance.

1.1.15. CLEAN UP. The contractor shall be responsible for leaving the appliance and area where work is performed neat and clean after repair or maintenance of each appliance. The contractor shall dispose of all containers, trash, and debris generated by this contract at no additional cost to the government.

1.1.16. APPLIANCE MISUSE OR ABUSE. When appliances are discovered in need of repairs and are in the opinion of the contractor, caused by abuse or misuse by other than the contractor's personnel, the contractor shall immediately notify the COR for determination of responsibility. In the event of abuse or misuse, the contractor shall submit in writing his proposal for repairs with a cost estimate. The total cost of each repair will be negotiated prior to actual repairs. Only the Contracting Officer (CO) can negotiate cost.

1.1.17. RESPONSIBILITY FOR DAMAGE. The contractor shall not hold the government responsible for any claim for damages caused to third party or property of third party arising out of:

a. The contractor's providing of defective appliances.

b. The contractor's defective maintenance of appliances.

c. The negligent operation of the appliances by the contractor or employees.

1.1.18. REMOVAL OF APPLIANCES. The contractor shall remove washers and dryers, with proper coordination through the CO and COR, in accordance with the phase out procedures identified in paragraph 1.1.18.

1.1.19. PHASE-OUT AND PHASE-IN. The phase-out of old washers and dryers and phase-in of new washers and dryers shall be performed to ensure no lapse in service. The contractor shall submit a phase-in installation schedule, see Para 1.1.4., to the CO/respective COR for review and approval. The phase-in schedule shall be typewritten and shall identify, by facility number and date, the scheduled installation of machines. The schedule shall be submitted five (5) business days before the start of the phase-in period. Delivery and installation of all new washers and dryers shall be performed in accordance with the approved phase-in schedule and shall be completed no later than sixty

(60) calendar days after start of phase-in. The contractor shall ensure washers and dryers are fully operational in a facility before proceeding to install machines in another facility. The new contractor shall coordinate installation of new machines with the incumbent contractor removing the old machines in order to maintain washer and dryer capacity in each new facility during each day of the phase-in and phase-out period. The government will provide a copy of the phase-in schedule to the incumbent contractor. The incumbent contractor shall remove old washers and dryers in accordance with the phase-in schedule. The incumbent contractor will not be allowed to store any washers and dryers on base.

1.1.20. MATERIAL SAFETY DATA SHEETS (MSDS). All hazardous materials to be used in performance of this contract shall be coordinated through the COR. A copy of all Manufacturer specific MSDS's shall be furnished to the COR a minimum of ten (10) calendar days prior to the delivery of the materials. This includes any material in a liquid, gel or semi solid state. Additionally, the size and/or weight of each container must be provided with the

MSDS. The Installation Hazardous Materials Program (IHMP) Management Office, located in BLDG 4196, is responsible for the approval of Hazardous Material.

1.1.21. DRYER VENT CLEANINGS. The contractor shall monitor, schedule and clean exhaust ducts quarterly to ensure ducts do not fill with lint, are clean and lint/dirt free and do not pose a fire/safety hazard. The contractor shall contact applicable facility manager to schedule cleaning services and provide a copy of schedule to COR at least ten (10) calendar days before services are performed. The contractor shall thoroughly clean all areas of the interior and exterior of the dryer exhaust ducts and adjacent areas so that all traces of dirt, mildew, lint, etc., are removed. The contractor shall clean all components to the dryer exhaust system to include exhaust caps, grills, louvers, bird/insect screens, rain caps, exhaust fan hoods and the exterior of ducts if exposed. The contractor shall clean the floor area around all dryers, specifically behind the dryers where lint and trash accumulate. Floor shall be cleaned of all dirt, lint, grime, trash, etc.

1.1.22. LAUNDRY TRAILER. The contractor shall provide the installation of a laundry trailer on the 1100 block of Fort Sam Houston. The laundry trailer shall have enough washer and dryer connections to accommodate a total of

10 top load washers and 10 electric dryers. Contractor will pay electric bill and shall be reimbursed at the end of every month. Contractor shall be responsible for all maintenance cost

1.2 GENERAL INFORMATION

1.2.1. QUALITY CONTROL. The contractor shall develop and maintain a Quality Control Program (QCP) to ensure services are performed in accordance with the PWS. The contractor shall develop and implement procedures to track, identify, prevent, remedy and ensure non-recurrence of defective service. The contractor is responsible for the QCP to include updates to ensure compliance with the PWS. Contractor’s QCP shall be summarized and provided to the Contracting Officer in the form of a Quality Control Plan within ten (10) calendar days after award of contract.

1.2.2 QUALITY ASSURANCE. The government will evaluate the contractor’s performance periodically throughout the life of the contract. The government representative will also evaluate the contractor’s performance through intermittent on-site inspections of the contractor's quality control program and receipt of complaints from base personnel. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. Likewise, the government may decrease the number of quality control inspections if performance dictates. The government will also receive and investigate complaints from various customers located on the installation. The contractor shall be responsible for initially validating customer complaints. However, the Contracting Officer shall make final determination of the validity of customer complaint(s) in cases of disagreement with customer(s).

1.2.3. GOVERNMENT REMEDIES. The contracting office shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items (May 1997), for contractor’s failure to correct nonconforming services.

1.2.4 HOURS OF OPERATION. The contractor shall schedule all work between the hours of 7:30 a.m. 4:30 p.m.

Monday through Friday. The contractor is not required to provide service on recognized federal holidays. The contractor will be required to respond to all emergency service in accordance with paragraph 1.2.4.1 of this PWS.

Period of Performance is 01 April 2013 – 31 March 2014 with three (3) one (1) options’ years leading through 31

March 2017. The contractor is not required to provide scheduled service on the following holidays:

New Years Day, January 1st

Martin Luther King's Birthday, 3rd Monday in January

Presidents Day, 3rd Monday in February

Memorial Day, Last Monday in May

Independence Day, July 4th

Labor Day, 1st Monday in September

Columbus Day, 2nd Monday in October

Veterans Day , November 11th

Thanksgiving Day, 4th Thursday in November

Christmas Day, December 25th

NOTE: Holidays falling on a Saturday will be observed on the preceding Friday. Holidays falling on a Sunday will be observed on the following Monday.

1.2.4.1 EMERGENCY SERVICES. The contractor shall be available within four (4) hours to provide emergency service between the hours of 7:30 a.m. and 4:30 p.m., Monday through Friday, except for Federal Holidays. The contractor shall provide a name and phone number to the COR upon contract award who will be responsible for responding to emergency service. Determination of emergency service is when 50% of the washers or 50% of the dryers are out of service per locations.

1.2.5 SECURITY REQUIREMENTS. As prescribed by the AFFAR 5352.242- 9000, Contractor access to Air

Force installations, the contractor shall comply with the following requirements:

a. The contractor shall obtain base identification and vehicle passes for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract.

b. The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of contractor employees needing access to the base. The authorized contracting officer will endorse the request and forward it to the issuing base pass and registration office or security forces for processing. Contractors will present government (state or federal) issued ID, and INS Form 9 (I9), before being issued a pass to enter the installation. At the time of issuance, a Wants and Warrants check is conducted for every contractor. To obtain a vehicle pass contractor employees shall provide a valid driver's license, current vehicle registration, and valid vehicle insurance certificate.

c. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

d. Upon completion or termination of the contract or expiration of the identification passes, contractors shall ensure that all base identification passes issued to contractor employees are returned to the issuing office.

If a contractor employee has been terminated, the pass will need to be retrieved and returned to Security

Forces so that employee does not have base access. If the pass is not retrieved then SF will need to be notified so base access is not allowed.

e. Failure to comply with these requirements may result in withholding of final payment.

Vehicles owned by the contractor with the company names permanently printed on them are not required to obtain a pass as long as a current work order is presented at the time of entry. Personnel employed by the contractor must get a pass for their privately owned vehicles at Bldg. #367, Visitor Reception Center, with proof of following:

A phone number for sponsor on base

1.2.5.1. SUITABILITY INVESTIGATION. If the contract is performed on other than normal duty hours, more than 30 days, or within a controlled area, the contractor is responsible for ensuring employees report to Bldg. #

1032, Visitor Reception Center to present their Form 1-9 (Employment Eligibility Verification).

If more than 180 days is required a NAC-I 85P background investigation must be initiated in order to extend a pass or to obtain a temporary Civilian Identification Card.

Contractor personnel shall successfully complete as a minimum, a National Agency Check with Inquiries

(NA CI), before operating government furnished workstations that have access to Air Force e-mail systems.

In addition, Contractor personnel who require routine access to the installation in excess of six months will require a NACI. The contractor shall comply with the DoD 5200.2- R, Personnel Security Program, AFI

33-204, Information Assurance (IA) Awareness Program. and AFI 33-202, Computer Security, requirements. NACI clearance requests are initiated using the Standard Form (SF) 85P and are submitted to 12 FTW/IP through the Unit Security Manager.

1.2.5.2 UNESCORTED ENTRY TO RESTRICTED AREAS. When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air

Force Installation Security Program, DoD 5200.2-R, and AFI 31-501, Personnel Security Program Management, as applicable. Contractor personnel shall be the subject of a favorably completed NACI investigation to qualify for unescorted entry to a restricted area. In addition, the Air Force shall submit NACI investigations for contractor employees at no additional cost to the contractor.

1.2.5.3 LISTING OF EMPLOYEES. The contractor shall maintain a current listing of employees. The list shall include employee's name, social security number, and date of investigation if service work involves unescorted entry to Air Force restricted or other sensitive areas designated by the installation commander. The list shall be validated and signed by the company Facility Security Officer (FSO) and provided to the contracting officer prior to the contract start date. Updated listings shall be provided when an employee's status or information changes.

1.2.5.4. FREEDOM OF INFORMATION ACT PROGRAM (FOIA). The contractor shall comply with DoD

Regulation 5400.7-R, DoD Freedom of Information Act Program, requirements. The regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding For

Official Use Only (FOUO) material. The contractor shall comply with AFI 33-332, Air Force Privacy Act Program, when collecting and maintaining information protected by the Privacy Act of 1974 authorized by Title 10, United

States Code, Section 8013. The contractor shall remove or destroy official records only in accordance with AFMAN

33-363, Management of Records, or other directives authorized in AFI 33-322, Records Management Program.

1.2.5.5. REPORTING REQUIRMENTS. The contractor shall comply with AFI 71-101, Volume- 1, Criminal

Investigations, and Volume-2, Protective Service Matters, requirements. Contractor personnel shall report to an appropriate authority, any information or circumstances of which they are aware may pose a threat to the security of

DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment and as required thereafter.

1.2.5.6. PERSONNEL SECURITY. Contractor personnel requiring unescorted entry to controlled or restricted areas designated by the installation Commander shall comply with base access requirements. These requirements are contained in AFI 31-101, for installation access and AFI 31-501, for suitability determinations.

1.2.5.7. ADDITIONAL SECURITY REQUIREMENTS. In accordance with DoD 5200.1-R and AFI 31-401, the contractor shall comply with AFI 33-202, Computer Security; AFI 33-203, Emission Security (EMSEC) Program;

AFI 33-204, Information Protection Security Awareness, Training, and Education (SATE) Program; applicable

AFKAGs, AFIs, and AFSSIs for Communication Security (COMSEC); and AFI 10-107, Operations Security

(OPSEC) Instructions. The contractor will comply with DOD Standard 22 – Level l AT Awareness training and associated tasking IAW AFI 10-245, Antiterrorism Program standards

1.2.5.8. PHYSICAL SECURITY. Areas controlled by contractor employees shall comply with base Operations

Plans/Instructions for FPCON procedures, Random Antiterrorism Measures (RAMS) and local search/identification requirements. The contractor shall safeguard all government property, including controlled forms, provided for contractor use. The contractor shall comply and implement local base procedures for entry to Air Force controlled and restricted areas.

1.2.5.9. KEY CONTROL. The contractor shall establish and implement key control procedures in the Quality

Control Plan to ensure keys issued to the contractor by the government are properly safeguarded and not used by unauthorized personnel. The contractor shall not duplicate keys issued by the government. Lost keys shall be reported immediately to the contracting officer. The government replaces lost keys or performs re-keying. The total cost of lost keys, re-keying or lock replacement shall be deducted from the monthly payment due the contractor.

1.2.5.10. GOVERNMENT AUTHORIZATION. The contractor shall ensure its employees do not allow government issued keys to be used by personnel other than current authorized contractor employees. Contractor employees shall not use keys to open work areas for personnel other than contractor employees engaged in performance of their duties, unless authorized by the government functional area chief.

1.2.5.11. LOCK COMBINATIONS. The contractor shall establish procedures in local Operating Instructions (OI) ensuring lock combinations are not revealed to unauthorized persons and ensure the procedures are implemented.

1.2.5.12. TRAFFIC LAWS. The contractor and their employees shall comply with base traffic regulation

1.2.5.13. CONTRACTOR MANPOWER REPORTING LANGUAGE

“The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the 502D Civil Engineering Squadron via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

Reporting inputs will be for the labor executed during the period of performance for each

Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk.”

*Reporting Period: Contractors are required to input data by 31 October of each year.

However, due to the delayed implementation, input of FY12 data is required by 31 Mar 13.

Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the

Army CMRA link at http://www.ecmra.mil.

3. GOVERNMENT FURNISHED UTILITIES AND SERVICES: The government shall provide without cost to the contractor, the utilities necessary to accomplish the tasks specified in PWS. The government shall provide without cost to the contractor, security police and fire protection.
