About this file

This is a Past Performance Questionnaire for solicitation FA301026R0009 issued by the 81st Contracting Squadron at Keesler Air Force Base, Mississippi. The questionnaire requests references to evaluate contractor performance on removal and replacement of basketball and racquetball courts located within Blake Gym at Keesler AFB. Completed questionnaires must be submitted via email to alice.townsend-peden@us.af.mil no later than 11:00 AM EST on May 22, 2026, directly from a corporate or government email account.

The questionnaire requires respondents to provide offeror information, contact details, and information about the referenced contract including contract number, value, and period of performance. Evaluators must rate contractor performance across five categories: Quality of Work, Schedule (timeliness and ability to resolve timeliness problems), Management (ability to manage all aspects of work), and Regulatory Compliance (safety regulations, Davis-Bacon Act, and other labor standards). Each category uses a six-point scale: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable. The questionnaire also requests an overall performance rating and provides space for detailed comments explaining specific performance details. Additional pages may be attached if needed for comprehensive responses.
