About this file
This is a Past Performance Questionnaire for Solicitation No. FA301026R0004, issued by the 81st Contracting Squadron at Keesler Air Force Base, Mississippi, for the repair of Foster Manor Building 6965 HVAC systems. The questionnaire must be completed by corporate or government email and submitted to SrA Tonny Mabonga at tonny.mabonga@us.af.mil by 2:00 PM CST on April 27, 2026. The questionnaire evaluates contractor performance across five key categories: Quality of Work, Schedule/Timeliness, Management, Regulatory Compliance, and Overall Rating. Evaluators must rate performance using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory) or mark Not Applicable, with space for detailed narrative comments. The form requests information about the referenced contract including contract number, value, period of performance, and a brief description of work. References must provide their name, title, agency/company, phone number, and email address, and may designate an alternate point of contact if unavailable for verification on April 27, 2026. This is a template document requiring completion by past performance references rather than submission of substantive deliverables.
