SUMMARY OF CHANGES
SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS
The following have been modified:
L-900 L-900. Proposal Requirements
NOTE: Failure to comply with the terms and conditions of the RFP, including a requirement to provide information requested by the Government to evaluate an offeror for award, may make an offeror unacceptable for award.
A. Proposals shall be submitted in two (2) separate sections as set forth below:
PAGE NUMBER
VOLUME LIMITS OF COPIES DESCRIPTION
I N/A 2 Completed RFP II N/A 2 Present/Past Performance Information
Proposals should comply with the following format:
(1) Print should be no smaller than a font size of 12 and should be on 8 1/2 x 11 inch paper. Charts, graphs or spreadsheets may be on fold-out pages, but must fold within the page size specified and will count as one page.
(2) Margins shall be no smaller than 1 inch.
(3) Each page within a section shall be numbered consecutively.
(4) Each page containing proprietary information should be so marked.
(5) Each page should contain the following legend at the bottom of each sheet:
SOURCE SELECTION INFORMATION--SEE FAR 2.101 and 3.104
FOR OFFICIAL USE ONLY
(6) Submissions of the completed RFP, Volume I, and past performance information, Volume II, shall be submitted in written form (1 copy) and CD-R or DVD-R (write once/read only) (1 copy) that is “closed” so that no further writes can be made to the media. CD-R/W or DVD-R/W media types are not acceptable.
Media must be virus scanned by the offeror prior to submitting to the Government.
B. Communications: Exchanges of source selection information between Government and offerors will be controlled by the Contracting Officer. Email may be used to transmit such information only if the email can be sent encrypted, and must include “Source Selection Information – See FAR 2.101 and 3.104” in the subject line of the email. Otherwise, source selection information will be transmitted via direct mailing or facsimile. In order to facilitate the sending and receiving of encrypted emails, offerors must use MS Outlook email configured to support encryption or a different email product that is S/MIME compatible and configured to support encryption. If you intend to submit your source selection information via encrypted email, you will need to contact the Buyer/Contracting Officer indicated on the face page of the solicitation prior to that first submittal in order to exchange certificates used for encryption. To insure the process is working correctly, send a test encrypted message first (without including any source selection information).
C. Volume I will consist of the completed and signed RFP with a cover letter delineating any exceptions taken to the RFP terms and conditions. However, offerors are cautioned that any noncompliance with the terms and conditions of the RFP may cause their proposal to be determined unacceptable. Offerors shall ensure that all clauses and provisions that require “fill-in” information are appropriately completed, including the proposed prices associated with the contract line items in the Schedule of the RFP. The Government intends to make an award without discussions. Discussions, if necessary, will be conducted in accordance with FAR 15.306. Volumes I and II of an offeror’s proposal must be received by WR-ALC/PKOC, 215 Page Road, Suite 131 Robins AFB, GA 31098-1672, Attention: Justin Stewart / Juanita S. Edmonds-Duke, no later than 21 January 2011, 4:00 PM, Eastern Standard Time. However, the Government requests that Volume II be received at least ten (10) calendar days prior to the required due date for proposals. Failure to submit Volume II by the earlier requested date will not result in offeror disqualification.
D. Volume II - Present and Past Performance: An offeror shall provide present and past performance information in a separate volume (i.e., 3-ring binder) labeled "Volume II - Present and Past Performance Information."
(1) An offeror shall provide the present/past performance information requested in the attached FACTS Sheets (See RFP Attachments VII, VIII, and IX) for ALL relevant work for which (a) the offeror was responsible as the prime contractor and (b) the offeror was responsible as a subcontractor. The offeror's present and past performance information may include data on efforts performed by the offeror’s other divisions or corporate management if such resources will be used in the performance of this proposed effort. The Government has elected NOT to evaluate the present/past performance of an offeror’s key personnel or potential subcontractors. Therefore, the information provided on the FACTS Sheets must be for relevant work for which the offeror itself was responsible as a Prime Contractor or Subcontractor.
(2) Each of the attached FACTS sheets must contain relevant present/past performance information for a specific twelve month time period: Effort Period 1 (RFP Attachment VII): 21 Dec 2009 – 20 Dec 2010; Effort Period 2 (RFP Attachment VIII): 21 Dec 2008 – 20 Dec 2009; Effort Period 3 (RFP Attachment IX): 21 Dec 2007 – 20 Dec 2008.
(3) If the relevant work was performed as a delivery or task order issued against an Indefinite Delivery/
Indefinite Quantity or Requirements Contract, as defined in FAR subpart 16.5, or against an equivalent type contract awarded by other than the Federal Government, then provide present/past performance information for each delivery/task order performed during the applicable Effort Period on a separate row. If the relevant work was NOT performed as a delivery or task order as described above, then provide present/past performance information for each relevant contract performed during the applicable Effort Period on a separate row. (See FACTS Sheets for examples.)
(4) All present/past performance information must be provided using the attached FACTS Sheets. Offerors may not vary the format of these FACTS Sheets except that each row of cells may be enlarged vertically and there is no limit to the number of rows that may be used. See RFP Attachment XVI, for additional FACTS Sheet instructions.
(5)The FACTS Sheets require an offeror to state whether FAR 52.219-8 was included in each relevant contract and/or order and, if so, whether the offeror complied with the policy stated in this clause when performing the relevant contract and/or order.
(6) The offeror shall focus its FACTS Sheet responses so that they clearly correlate present and past performance with the requirements of this RFP. The offeror shall provide the most current information for the Points of Contact (POCs) identified on the FACTS Sheets to include telephone number AND email address. The Government intends to obtain information from the applicable POC relating to the performance of each effort identified on Attachments VII, VIII, and IX.
(7) If problems were encountered during the performance of the identified relevant contracts, provide an explanation, in a separate document, that demonstrates the offeror's ability to isolate the root causes of problems and includes a description of programs or actions taken to resolve those causes. Problems not addressed by the offeror, but found by the Government during the evaluation of the information in this volume, will be assumed to still exist.
Note: In the case of Contractor Performance Assessment Report System (CPARS) or Construction Contractor
Appraisal Support System, if your input has already been provided and the rationale/circumstances have not changed, DO NOT repeat them here.
(8) In the event that commercial contracts are presented as present/past performance sources of information, a client authorization letter shall be issued by the offeror to those commercial POCs requesting/authorizing them to provide information to the Government relative to the offeror’s past performance on those commercial contracts. A sample client authorization letter is attached to this RFP (see RFP Attachment X). The offeror is required to send the client authorization letter(s) to each POC on commercial contracts. A separate copy of client authorization letter(s) for each commercial contract shall be included in the offeror’s Present/Past Performance submission for the Government’s use.
