About this file
This is an instructional guide for the Fermilab Onsite Access Request Form for Business Visitors. The form is designed for visitors requiring onsite access to Fermilab for business purposes—such as events, job interviews, or meetings—without computing access needs and for stays of less than seven days. The form requires visitors to provide comprehensive personal information, including legal name, date of birth, citizenship status, and country/region of birth. Non-U.S. citizens must answer additional questions regarding Legal Permanent Resident status and provide visa documentation (passport, I-94, LPR card, ESTA paperwork, or B-1 visa).
The application process includes nine primary sections: worker type selection, verification of previous Fermilab access history, reason and dates of visit (with Fermilab affiliation and point of contact specification), professional class designation (such as procurement meeting or event attendance), personal demographic information, primary contact details (email and phone), home institution identification, acknowledgment of Community Standards Agreement and Export Control Policy, and document uploads. The guide specifies that all documents must be in PDF or JPG/JPEG format with file sizes under 10MB, and explicitly prohibits HEIC (Apple/iPhone) formats. Processing timelines include Point of Contact approval (1 day), Foreign National Access Program (FNAP) and Export Control (EC) review if applicable (2-4 weeks), FNAP processing (2-4 days), and issuance of an Informal Invitation Letter containing the Fermi ID number, approved access dates, and a QR code for security gate entry. For non-U.S. citizens on short-term visas (ESTA or B-1), FNAP processing cannot be completed until they are physically in the United States and have received an I-94 from Customs and Border Patrol.

Then click “Next”
Have you previously submitted a Fermilab Access Request form, or have you ever had a Fermilab ID badge or a Fermilab computing account?
We need to know if you have had access (computing and/or ID badge) with us before so that the systems don’t create a duplicate profile and Fermi ID number for you.
If you have had access in the past but do not remember your ID number, please email SOC@listserv.fnal.gov and we will be happy to give that to you.
If you have not had access with us before, select “No” and then click “Next.”
If you have had access with us before, select “Yes,” and then select how you want to link your previous profile/Fermi ID to this form.
The option that you pick from that drop-down will then prompt a new field for you to enter that information: email address, phone number or Fermi ID number (must be 5 digits plus V)
Enter your information (email, phone, ID number) and a pop-out window will appear with information associated with that email/phone/ID.
If this information is correct, click “Confirm.” You can also use the drop-down to pick a different profile.
If it is not correct, click “Cancel” and email SOC@listserv.fnal.gov for help.
Click “Next” at the bottom right when you are done
