About this file

This is an Installation Floater Affidavit form for ITB 26-B-39IF, a reconstruction project for the Florida Shores Boulevard Vehicular Beach Access Ramp in Daytona Beach Shores, administered by Volusia County, Florida, in partnership with the State of Florida Department of Environmental Protection. The affidavit serves as a notarized statement from the insurance broker of a contract bidder documenting efforts to obtain specialized insurance coverage required by the bid. The form requires the affiant to list three insurance companies contacted to obtain Installation Floater or Builder's Risk insurance policies with Flood coverage extensions that specifically cover flood damage resulting from storm surge caused by named and unnamed storms for beach and intracoastal waterway construction projects. The sealed bid submission deadline for ITB 26-B-39IF is April 9, 2026, at 3:00 p.m., with a non-mandatory pre-bid meeting scheduled for March 16, 2026, at 10:30 a.m., and a question submission deadline of March 26, 2026, at 11:59 p.m.

The affidavit is submitted by the contract bidder to document that the three contacted insurance companies informed the broker that they cannot provide the required Installation Floater and Builder's Risk insurance coverage with Flood coverage extensions for this project. This documentation appears to serve as evidence of good faith efforts to obtain the mandated insurance coverage. The form requires notarization with witness signatures and must be completed before the bid submission deadline. Award is anticipated in May 2026 to the lowest responsive and responsible bidder, with the total estimated project budget of $2,041,339 funded through FEMA sources requiring compliance with federal contract provisions including Davis-Bacon Act prevailing wage requirements and other federal contract standards.
