About this file
This is a New Jersey State Two-Year Chapter 51/Executive Order 117 Vendor Certification and Disclosure of Political Contributions form (Exhibit F) used by state agencies for contract procurement processes. The form requires vendors to provide comprehensive business entity information including full legal name, address, Federal Employer Identification Number, and detailed ownership structure based on business type (corporation, professional corporation, partnership, limited liability company, or sole proprietor). All officers, shareholders with 10% or greater ownership, equity partners, or LLC members must be listed depending on the business structure. The form serves as a two-year certification that can be used for recertification purposes.
The primary focus of this form is compliance with New Jersey's political contribution disclosure requirements, mandating vendors report all political contributions made during specific timeframes: 4 years for certain political organizations, 5.5 years for gubernatorial candidate committees and state/county political party committees, and 18 months for municipal political party committees and legislative leadership committees. Vendors must certify they have not made contributions that would bar contract awards and acknowledge ongoing reporting responsibilities during the contract term. The form requires submission to the contracting state agency and includes provisions for electronic submission to cd134@treas.nj.gov or mail delivery to the Chapter 51 Review Unit in Trenton, with the state agency forwarding copies while retaining originals for their records.
