About this file

Exhibit G is an evaluation tool used by the Texas Health and Human Services Commission (HHSC) for Request for Qualifications (RFQ) No. HHS0016750 to assess respondent proposals for providing specialized residential health services in Denton County. The solicitation seeks a qualified provider to deliver comprehensive health care, dental care, and 24-hour community-based residential services for an elderly, medically complex individual with moderate to severe intellectual disability. Key service requirements include comprehensive and functional assessments, specialized behavioral training, case management, crisis services, and development of individualized age-appropriate goals. All services must be delivered in person at the residential setting with no telehealth or remote delivery permitted. Written questions are due January 20, 2026, at 10:30 a.m., with responses posted by January 27, 2026. Courtesy review submissions for the Historically Underutilized Business (HUB) Subcontracting Plan (HSP) are due January 30, 2026, at 5:00 p.m., and final solicitation responses are due February 12, 2026, at 10:30 a.m. Central Time, with an anticipated award date of April 2026. The initial contract term is two years, with HHSC holding options to extend for up to three additional years plus potential six-month extensions for continuity of service or transition purposes.

The evaluation tool weights qualifications and experience at 65 percent, comprising two subcriteria: respondent's experience in residential, medical, behavioral, nursing, transportation, and habilitation services (35 percent) and key staffing profiles and resumes (30 percent). The project work plan comprises the remaining 35 percent of the evaluation. Scoring uses a 10-point scale ranging from unacceptable to exceptional, with respondents required to demonstrate a minimum of five years of operational experience as either a Home and Community-based Services provider or a licensed Home and Community Support Services Agency and provide at least three references from similar contracts within the last three years. Historical compensation for similar transactions has been approximately $2,346,917.00 per State Fiscal Year, though HHSC makes no guarantee of volume, usage, or compensation. All responses are subject to rejection if a completed HSP is not submitted, with respondents required to select from five HSP method options including utilizing certified Texas HUBs, mentor-protege relationships, meeting HUB goals through subcontracting, solicitation methods with HUB outreach, or self-performing all work. Required insurance coverage and a Data Use Agreement are mandated, with all confidential information required to remain within the United States, and respondents must be authorized to conduct business in Texas prior to award.
