The contract requires a qualified provider to deliver comprehensive, community-based residential and health services for an elderly, medically complex individual with moderate to severe intellectual disability residing in Denton or Tarrant County, Texas. Services must commence within five calendar days of the contract effective date and include 24-hour residential support, nursing care, behavioral interventions, case management, and transportation. The contractor must assume or secure an appropriate three-bedroom, two-bathroom residential lease in a community setting that maintains a home-like appearance without institutional qualities. The SOW specifies detailed requirements for residential setting compliance, direct care staffing protocols (limiting service providers to 16 consecutive hours with mandatory eight-hour rest periods), comprehensive nursing assessments conducted annually by a registered nurse, and an interdisciplinary team approach involving case managers, social workers, behavioral support professionals, habilitation specialists, and residential managers. All staff must meet specific qualification requirements including background checks, criminal history clearances, and specialized training. The initial contract term is two years with HHSC holding options to extend for up to three additional years plus potential six-month extensions for service continuity or transition purposes. Final solicitation response submission is scheduled for February 12, 2026, at 10:30 a.m. Central Time, with an anticipated award date of April 2026.

The contractor must submit monthly invoices within 20 calendar days following the service delivery month, supported by documentation including general ledgers, invoices, contracts, and receipts. Historical compensation has approximated $2,346,917.00 per State Fiscal Year, though HHSC makes no guarantee of volume or payment level. Non-reimbursable expenses include standard business operations costs, per diem, travel expenses, offsite office space, prepared meals not for exclusive individual consumption, gratuities, and vehicle tolls except in medical or behavioral emergencies. The contract includes strict compliance requirements for criminal history checks (annual fingerprint checks and monthly registry clearances), abuse and neglect reporting protocols mandating notification to the Department of Family and Protective Services within one hour of suspected abuse, and comprehensive documentation standards for all service delivery. Respondents must demonstrate a minimum of five years of operational experience as a Home and Community-based Services provider or licensed Home and Community Support Services Agency and provide at least three references from similar contracts within the last three years.
