About this file

This document is an instructional guide for the Deliverable Acceptance Form process for the Department of Family and Protective Services (DFPS) in Texas regarding Independent Verification and Validation Services for Case Management Transformation. The procedures establish a formal workflow wherein the Contractor's Contract Manager submits deliverables to the DFPS Program Manager and/or Contract Manager, who then forwards each deliverable along with a Deliverable Acceptance Form to the DFPS Approver. The DFPS Approver must formally approve or reject each deliverable within five business days, with no default acceptance or conditional approvals permitted. All deliverable-specific acceptance criteria are detailed in Exhibit S of the contract, and approvals may be transmitted via email with the approver's typed name, provided the email is saved as a text document and pasted into the acceptance form.

Should a deliverable be disapproved, the DFPS Approver must provide detailed explanations of all errors and omissions in the initial rejection notice. If disapproval is based on factors not identified in the original acceptance criteria, the DFPS Approver must submit a change request to amend the criteria, and the DFPS Contract Manager or Program Manager will reschedule the deliverable due dates accordingly. The DFPS Contract Manager maintains an acceptance log documenting each deliverable's submission date, approval or rejection status, and action date, with each deliverable identified by its unique Deliverable ID from the contract. Verbal approvals are not acceptable under any circumstances.

Exhibit G-1 - Deliverable Acceptance Form Instructions

1. The Contractor Contract Manager sends the Deliverable to the DFPS Program Manager and/or DFPS Contract Manager.

2. The DFPS Contract Manager will deliver a Deliverable Acceptance Form (as shown in Exhibit G, Deliverable Acceptance Form) to the DFPS Approver with each deliverable. The Deliverable Acceptance Form will identify the Deliverable(s) and the acceptance criteria. The DFPS Contract

Manager or Program Manager will sign the acceptance form indicating that the Deliverable meets all requirements.

3. Upon receipt of the Deliverable and Deliverable Acceptance Form from Contractor, the DFPS Approver has five (5) Business Days to formally approve or reject the Deliverable. The form must be marked approved or disapproved and signed by both Parties.

NOTE: There is no acceptance or rejection by default. “Conditional” approvals are not allowed, as they would not be handled any differently than rejection of approval.

4. If the Deliverable is disapproved, a detailed description of why it was rejected must be included with the form. All errors and omissions must be detailed in the first rejection.

5. If the disapproval is based on factors that were not identified in the acceptance criteria, the DFPS Approver must submit a change request to add the factors to the acceptance criteria and the DFPS

Contract Manager or Program Manager will reschedule the due dates for the Deliverable based on the change request.

6. E-mail approval with the DFPS approver’s name typed on the form is an acceptable form of transmittal. When e-mail approvals are used, the e-mail with the DFPS approval should be saved as a text document and pasted into the acceptance form at the end of the form. Verbal approvals are not acceptable.

Criteria for Acceptance:

Deliverable/work product specific acceptance criteria have been included in the Exhibit S, Deliverables. Formal approval of this document constitutes acceptance of these criteria.

Acceptance Log

The DFPS Contract Manager will maintain an acceptance log to document the delivery and approval of each Deliverable. The acceptance log will include the following information:

1. Record Deliverable ID. Each Deliverable has a Deliverable ID from the Contract.

2. Deliverable – brief identification of the Deliverable which may include the cross reference from the Project Work Plan or Exhibit S, Deliverables.

3. Date Submitted – the date the Contractor Project Manager presents the Deliverable to the approving authority for acceptance.

4. Approved or rejected – indicating whether the Deliverable is approved or rejected.

5. Date of Action – Date that the approval or rejection took place.

If there is a large number of Deliverables, the form may be coded or shaded to indicate completion of

Deliverables.
