About this file
This is an Exhibit D Special Conditions document for Pinellas County Human Services establishing contractual requirements for a Rapid Rehousing Program fiscal administrator and service provider collaborative. The agreement outlines comprehensive operational, financial, and administrative conditions that contractors must follow throughout the contract term. Key service requirements include participation in the Housing First philosophy and coordinated entry system, operation under a low-barrier model prioritizing rapid housing placement, and participation in the Pinellas Homeless Management Information System (PHMIS). Contractors must maintain continuity of operations plans with annual reviews and be prepared to support emergency disaster response efforts. All contractors are required to list programs in the First Contact online database, accept referrals, and provide timely updates on program eligibility, capacity, and availability within thirty calendar days of any changes.
The financial terms require monthly invoicing within thirty days of month-end, with the County providing reimbursement within forty-five days of receipt of proper invoices in accordance with the Local Government Prompt Payment Act. Contractors must comply with the County's travel reimbursement policy (or their own if lower) and reinvest any Program Generated Income back into the program with tracking and documentation. The County reserves the right to reduce total awards on a prorated basis if contractors fail to provide services or draw funding for two or more consecutive months. Personnel requirements mandate submission of names and qualifications of key staff within thirty days of execution, with notification of any changes to CEO, COO, CFO, CITO, or integral program positions within three business days. Contractors must provide annual financial audits or Form 990s within thirty days of completion, maintain all required licenses and insurance certificates, and retain all records for five years after final payment. The County retains monitoring and audit rights with full access to sites, personnel, and programmatic documents.
