9. The contractor shall follow VA naming conventions attached at both the patch panel and wall plate. All labeling shall be easily visible and printed, handwritten labeling is prohibited.

2. SECURITY REQUIREMENTS

Below are requirements for a Facility Security Level 2 (KOW notes that links to all Handbook references below need to be included in this SOW)

In addition to the items listed in the SOW. Please add the following:

In the IT area/room, security requirements should follow items required in VA Handbook 0730/4, Appendix B (ex. If this is specified as an IT Telecomm/Data Connection Closet, walls should conform, doors and door locks should conform, no “Other Room Access Means” and “Special Key Control” should conform.)

The “Commercial Electrical Power Connections” located at the rear of the building should have appropriate perimeter barriers as specified in VA Handbook 0730/4, Appendix B.

If Pharmacy management is confirming that the room containing the Omnicell is to be specified as a “Pharmacy Drug Storage Room”, guidelines/requirements set in VA Handbook 0730/4, Appendix B should be followed. It is our understanding that there will be an Omnicell contained in this room, but no narcotics or controlled substances will be stored within this Omnicell. If this is the case, Pharmacy may specify this room as a “Pharmacy Inventory (Medical Supply)” area and if so, guidelines/requirements set in VA Handbook 0730/4, Appendix B should be followed.

All “Treatment Rooms” should follow guidelines/requirements set in VA Handbook 0730/4, Appendix B. All interior cabinets should be locking.

The clinic should have a Video surveillance system. We specifically recommend Avigilon brand camera systems to be configured so that VA Police Dispatch in Dallas can monitor these cameras. The ISC (Interagency Security Committee) manual specifies that VSS is required and specifies where they are required.

IDS (Intrusion Detection System) is existent at this facility but appears to be inactive. The ISC (Interagency Security Committee) specifies that this is required as well with specifics.

Panic Duress capabilities were not observed, and it is highly recommended in key locations. Our Lynx program will be implemented for an “in house” alert system between employees’ computers but will not alert the Police. There should be a panic/duress system at this location to alert local Police during emergencies.

ISC (Interagency Security Committee) requires securing all perimeter doors with “non-removable hinges and high security mechanical or electronic locks”.

Electronic Physical Access Control is required on all employee entrances. Considering employees tend to enter all entrances, we recommend all entrances being equipped with this technology. The access should be governed by use of a PIV. Entrances open to the public would be placed on a timer, eliminating the need to utilize a PIV during business hours if desired.
