This is a Service Level and Deliverable Matrix document for the SCEIS Modernization contract (Solicitation No. 5400026039), establishing performance expectations and deliverable requirements for the service provider. The document details critical service levels across eight categories including service availability at 99.99% expected level and 99.95% minimum level, incident resolution times for Priority 1 and 2 incidents at 99.00% expected and 98.00% minimum compliance, root cause analysis delivery at 98.00% expected and 97.00% minimum, and service request fulfillment at 98.00% expected and 96.00% minimum performance. Additional critical service levels address solution proposal delivery, project deliverable completion, data protection backups, and batch processing, each with defined expected and minimum service levels measured on a monthly basis. Seven key service levels are also established, including response time for applications, change management effectiveness, invoice dispute resolution, license and maintenance renewal timeliness, corrective actions, incident resolution for Priority 3 and 4 incidents, and patch compliance, each with six-month commencement timelines or immediate applicability. One-time deliverables include the Transition Plan and Transition Milestone Document due 60 days prior to transition activities with a $15,000 credit, the Operational Readiness Assessment due 30 days before commencement with $15,000 credit, Service Management Manual documentation across three phases with $10,000 credits each, disaster recovery planning deliverables with $10,000 credits, and System Security Plans with $10,000 credits.

Recurring deliverables include an Annual Technology Plan and Roadmap due annually with $10,000 payment credits, quarterly refresh plans with associated completion reports, annual refresh plans addressing equipment and software renewal cycles, third-party information security attestations due 30 days prior to data storage implementation and annually thereafter at $10,000 credit, annual customer satisfaction improvement plans at $10,000 credit, disaster recovery test plans and schedules at $10,000 annual credit, training documentation for system upgrades and enhancements prior to production deployment, quarterly technology plan updates, and corrective action plans within 30 business days of identified failure triggers at $15,000 credit. The Critical SLAs carry an at-risk percentage of 12% with a pool percentage of 200% available for allocation, with service level credits ranging from 1.80% to 4.20% of the invoice depending on the specific service level. All service levels are subject to continuous improvement except for specific exceptions noted in the documentation, and measurement windows are established on a monthly basis for tracking compliance and applying credits for non-performance.
