About this file
This document is a Respondent Questionnaire and Evaluation Criteria form issued by the University of Texas at Austin for the structural and electrical repairs project at Central Chilling Station No. 3 (CS3), Contract No. 26CSP006. The RFP requires comprehensive responses across nine evaluation criteria, with qualifications constituting a maximum of 65 points of the overall 100-point scoring system. Respondents must provide detailed general information including business structure, financial data, bonding capacity, litigation history, and employee relationships with university staff. The evaluation criteria assess the contractor's ability to provide construction services, qualifications of the proposed construction team, technical and management competence with structural and electrical repair projects, past experience with the University of Texas System, safety program and record, project execution plan and scheduling, quality control and commissioning program, capability to achieve project close-out, and warranty and service support program.
The questionnaire requires respondents to submit detailed information including resumes for all key personnel, subcontractor lists with performance history, customer references from at least three completed projects within the past five years, OSHA safety ratings and certifications, preliminary Critical Path Method schedules, quality control methodologies, and warranty service plans. Respondents must demonstrate experience on similarly complex facilities maintaining continuous operation during construction, such as universities, hospitals, laboratories, or semiconductor facilities. The document specifies that both the project superintendent and project manager must maintain current 30-hour OSHA certification and that key personnel must commit to the project for its duration unless an equally qualified substitute is approved by the owner. The form does not specify response due dates, award dates, or contract term information within the evaluation criteria document itself.

DART (Calendar Days Away / Restricted Time) Rate³
Number of Fatalities*
Number of OSHA Violations*
Worker’s Comp Experience Modification Rate (EMR)
