About this file
This document is a Texas Family Code Schedule related to Solicitation No. 601440000049657, which pertains to ineligibility to receive state grants, loans, or contract payments. The schedule requires respondents to disclose information about ownership and child support obligations, specifically mandating that any business entity with individuals holding at least a 25% ownership interest must provide names and social security numbers. The document outlines that the contract may be terminated and payment withheld if the provided certification is inaccurate.
The schedule is part of a broader compliance mechanism under Texas Family Code §231.006, which aims to identify and prevent individuals or entities with outstanding child support obligations from receiving state contracts. The document includes a Federal Privacy Act notice explaining that social security number disclosure is required to identify persons potentially owing child support, with assurances that such information will be kept confidential to the fullest extent allowed by Texas Family Code. Respondents must sign and return this schedule with their response, with failure to do so potentially resulting in the response being considered non-responsive.
