The purpose of this amendment is to clarify answer #5 and to provide the revised page 61 of the SIR.
First: Answer # 5 is clarified on page 2 of this amendment.
Second: Page 61 revision is hereby attached to this amendment, page 3.
Third: All other terms and conditions remain in full force and effect.

5 Q: For the J.23 submissions that ask for actual documents to support the various narratives, are these to be included in the J.23 document itself where it is requested?
A: No. The project documentation must be in a separate folder clearly marked as to which J.23 submission it pertains to. The project documents are vendor format and not included in the J.23 narrative page count. Vendor project documentation has its own page count.

9-21-16 Revised partners, or is in a joint venture it is important they specifically describe their relationship and work performed by each. Vendors that are newly formed businesses, without prior contracts or that do not possess relevant corporate past and present performance, may submit up to one (1) Past Experience History, Attachment J.23, on key personnel demonstrating their most recent past and present history relevant to this contract. The FAA will not consider performance on a newly awarded contract without a performance history or on any effort completed more than five (5) years prior to this contract’s solicitation date. However, the FAA may use data obtained from other sources in the evaluation of past performance and experience and contract agreements older than five (5) years.
(C) Relevant Past Experience Forms: The vendor can provide Relevant Past Experience History forms on up to six (6) different Application, as shown in Attachment J.23a, J.23b, J.23c, J.23d, J.23e, and J.23f within the past five (5) years. Vendor must submit a minimum of three different J.23 Applications of battery system work (including large UPS) using Past Experience History, Attachment J.23. The vender must attach relevant project documents to each Past Experience History, Attachment J.23, and clearly marking the applicable J.23 Application and Work Effort. The FAA will use data provided by each vendor in Volume II and may use data obtained from other sources in the evaluation of past performance and experience. Attachment J.23, in FAA format. Relevant Past Experience History forms are either 8-pages or 9-pages in length as determined by application, and are not to be lengthened; narrative must be confined to the space provided. Up to six (6) Relevant Past Experience History Attachment J.23 forms are allowed for a maximum page count of fifty-one
(51) pages. Only one J.23 submittal for each category (J.23a, J.23b, etc.) is allowed. Project documentation is in vendor format and vendor project documents are not counted against the J.23 narrative 51-pages. Each Relevant Past Experience History Attachment J.23 submitted must have a separate folder, clearly marked as to which J.23 form it pertains to, these folders must contain the requested project documentation for that J.23 submission. For each Attachment J.23 Relevant Past Experience History the vendor should include narratives and project documentation of the following battery system project work efforts:
1) Program Management.
a. Narrative: Provide a description of your Program Management effort as either prime contractor or subcontractor on this contract.
b. Document: Provide a copy of Program Plan and cost reports
2) Battery String Installation.
a. Narrative: Provide a description of your battery string replacement experience.
3) Program Management Cost Reporting.
a. Narrative: Demonstrate your experience providing customers with periodic cost reports that provide project tracking by delivery orders, completion and cost data on projects.
b. Document: Provide a copy of a typical Program management cost report completed by your company for your customers.
4) Installation Schedule.
a. Narrative: Explain your process used to develop an installation schedule that begins with initial customer order to final clean-up and inspection.
b. Document: Provide a copy of the Installation Schedule completed by your company or company’s key personnel for this project.
5) Site Survey.
a. Narrative: Explain how a site survey is used to collect information necessary to adequately coordinate and perform battery replacements. See paragraph
7.1.1 of Section-J, Attachment J.1 Statement of Work.
b. Document: Provide a copy of the site survey performed and Site Survey Report completed by your company or company’s key personnel for this project.
6) Seismic Analysis for Battery Rack.
a. Narrative: Explain the process and documentation needed to for a seismic report with calculations and anchoring recommendations.
