About this file

This document is a Proposer Information Form for the HOWTO Grant Program, which is a state and local contract opportunity. The form requires the proposer to provide information about their organization, including their name, website, address, tax ID, insurance coverage, and contact information. The form also asks the proposer to describe their ongoing actions to advance antiracism, equity, and inclusion, as well as the counties and populations they serve. Additionally, the form requests details about the proposed project, such as the estimated budget, timeline, collaborators, how workforce participants will guide the work, and which counties will benefit from the project. The form focuses on the proposer's qualifications and the proposed project's ability to address healthcare workforce needs, increase diversity, and advance health equity.

The document provides specific instructions and requirements for the proposer to complete and submit the form as part of the contract opportunity. It does not include pricing terms, set asides, incumbent information, or funding sources, as this appears to be a grant program rather than a traditional contract.
