About this file

This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) to provide guidance on bidding requirements for construction projects, specifically referencing ARFQ DEP2200000001 for Peeltree Road Portals. The document establishes mandatory requirements that bidders must comply with to avoid immediate disqualification, including attendance at mandatory pre-bid meetings, signature of required bidder instruction pages, submission of valid bid bonds, acknowledgment of addenda where stipulated as mandatory, timely bid submission prior to closing dates, and verification of non-debarment status at both federal and state levels. Additionally, bidders must demonstrate the absence of outstanding environmental enforcement actions and unresolved DEP vendor complaints. The form emphasizes that all potential bidders must thoroughly review the complete request for quotation and all associated bid documents to identify specific project requirements and bidding procedures.

The form identifies additional conditions that may result in bid disqualification prior to contract award, including outstanding debt to the state or political subdivisions, workers' compensation or unemployment compensation delinquency, and failure to obtain required bonds and insurance. Bidders must supply a valid West Virginia contractor's license number and signed drug-free workplace affidavits either with their initial bid submission or within one business day of DEP request. Subcontractor listings must be provided within one business day of bid opening or upon DEP request. The document serves as an informational checklist intended to highlight common bidding problems but explicitly does not constitute a complete list of all requirements, which are detailed in the full bid documents.
