About this file
This document is a 6-Month Asbestos Reinspection Report for the Jennifer Moreno Department of Veterans Affairs Medical Center located at 3350 La Jolla Village Drive, San Diego, California 92161, prepared by Millennium Consulting Associates. The reinspection was conducted in March-April 2024 as a follow-up to a 3-Year Asbestos Survey performed from June through September 2023, covering 15 buildings (1, 2, 9, 11, 12, 14, 16, 17, 18, 20, 23, 24, 25, 27, and 28) at the medical center. The report documents findings for Building 1 (Main Hospital), Building 2 (Power Plant), and Building 11 (Spinal Cord Injury Building), which contain known asbestos-containing materials (ACM), while the remaining buildings showed no known ACM presence.
Building 1, a seven-story, 855,000-square-foot facility constructed between 1969-1972, contains significant ACM including: approximately 1,400,000 square feet of dark grey spray-applied fireproofing at structural steel components and concrete deck (containing 4-30% asbestos) with 100,000 square feet estimated as damaged debris; 13,500 linear feet of Thermal System Insulation with 2-50% asbestos content; 12,500 square feet of brown duct mastic (2-15% asbestos); 112,778 square feet of twelve-inch resilient floor tile and black floor mastic throughout patient levels; and various drywall/joint compound materials. Building 2 contains 128 linear feet of green-wrapped thermal system insulation and four white elbows on backup boiler pipes. Building 11 contains black floor-tile mastic underneath twelve-inch vinyl tiles on the first floor. The report provides detailed hazard rankings and damage assessments for all ACM, with recommendations including development of a task-specific cleanup plan for ACM debris in interstitial spaces, compliance with Cal-OSHA and Federal OSHA standards during future renovations, regular inspection schedules for patient levels, and implementation of contractor requirements to prevent damage to remaining ACM during work activities.

Asbestos-Containing Material (ACM) regulations are published by the U.S. Environmental Protection Agency (EPA) and the State of California, Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA). A material is considered by the EPA to be ACM if at least one sample collected (by homogeneous group) is analyzed and asbestos is detected in an amount greater than one percent (>1%).
Both Federal OSHA and Cal/OSHA consider a material to be ACM if it is found to contain greater than one percent (>1%) asbestos. In addition, Cal/OSHA defines Asbestos-Containing Construction Material (ACCM) as a material containing greater than one-tenth of one percent (>0.1%) asbestos. Employers performing construction activities where ACCM is present must comply with all requirements of registration by Cal/OSHA for asbestos-related work and all applicable provisions of the California Code of Regulations, Title 8, Section 1529.
Summary tables containing sampling results, material descriptions, material locations, EPA categories and damage assessment for the entire ACM survey may be found in Section 4.
Recommendations Based on the data collected and referenced herein, Millennium recommends the following course of actions:
1. A task specific plan for the cleanup of ACM debris throughout the interstitial levels including the repair and encapsulation of damaged fireproofing at structural members, particularly at beams that are at a proximity to the catwalks and other paths of travel.
2. All Regulated Asbestos-containing Materials (RACM) that will be disturbed or affected by any future work shall be done in compliance with the asbestos National Emissions Standards for Hazardous Air Pollutants (NESHAP), and Cal-OSHA Asbestos in the Construction Industry Standard, 8 California Code of Regulations (CCR) 1529. Additionally, all Category I and Category II non-friable asbestos-containing materials that may become friable as a result of renovation/demolition work and that may be affected by the planned renovation shall be removed prior to renovation/demolition (or other activities resulting in the disturbance of these types of ACM) of the subject building in compliance with the San Diego Air Pollution Control District Rule 1206, National Emission Standards for Hazardous Air Pollutants (NESHAPS), and Cal- OSHA Asbestos in the Construction Industry Standard, 8 CCR 1529.
Millennium Project No. 22004.2200 -6- 22004.2200
3. Materials containing any detectable concentration of asbestos impacted by a planned renovation shall be handled in accordance with the Cal/OSHA Asbestos in the Construction Industry Standard, 8 CCR 1529 and the Federal OSHA 1926.1101.
4. All assumed asbestos-containing materials referenced herein shall be treated as asbestos-containing materials and managed accordingly, or – shall be sampled and analyzed for asbestos content prior to renovation.
5. A regular inspection schedule and assessment of all patient levels including the sixth-floor research level and Penthouse should be performed, directly related to the management of ACM flooring, wall systems and TSI especially in high traffic areas with potential for damage.
6. Implement contract requirements for contractors to reinforce existing regulations. This includes during work activities within the patient levels and research level that require the cleanup of any asbestos debris within the work areas or paths of travel. Under Cal/OSHA and Federal OSHA, asbestos hazards resulting from work activities shall be abated by the contractor who created or controls the source of the contamination (see 1926.1101(d)(2) and 1529(d)(2)).
7. Implement contract requirements to limit any attachment to the asbestos-containing fireproofing for any future utilities or equipment installation (e.g., ethernet cables, patch cables, electrical conduit, HVAC strapping, ladders, toe boards, scaffolding etc.). Also, ensure that contractors and other workers are not drilling, fastening, or coring into the asbestos-containing fireproofing overspray at the concrete deck during work.
8. An above ceiling access procedure should be developed for maintenance/engineering staff and for contractors when ceiling tiles are moved to access the above ceiling space.
9. A task specific plan for the cleanup of ACM debris throughout the interstitial levels including the repair and encapsulation of damaged fireproofing at structural members, particularly at beams that are within a close proximity to the catwalks and other paths of travel.
10. Continue to repair/replace damaged non-asbestos containing vinyl floor-tiles covering the asbestos containing floor-tile mastic beneath.
11. Special care should be taken when buffing, waxing and or cleaning flooring that is a known ACM and non-asbestos containing floor-tiles covering asbestos containing mastic. The speed of buffing should never exceed 300 rpm when buffing asbestos containing floor-tile.
In summary, building owners are responsible for how asbestos-containing materials, presumed asbestos-containing materials (PACM), and assumed asbestos-containing materials are handled at and within their buildings. This includes tenants, maintenance and custodial workers, contractors, and others. Building owners must also verify asbestos remaining at or within their buildings is properly managed and maintained in a nonhazardous fashion. This includes notification, disposal, labeling, training and record keeping requirements, within EPA/OSHA regulations.

1.0 INTRODUCTION
Millennium Consulting Associates (Millennium) was retained by the Department of Veterans Affairs to conduct a 6-Month Asbestos Reinspection of fifteen buildings comprising the Jennifer Moreno Veterans Affairs Medical Center. The survey included buildings 1, 2, 9, 11, 12, 14, 16, 17, 18, 20, 23, 24, 25, 27, and
28. The Jennifer Moreno Medical Center is located at 3350 La Jolla Village Dr, San Diego, California 92161.
The Hazardous Materials survey was conducted from June 2023 through September 2023. Of the fifteen buildings included in the inspections only Building 1 (Main Hospital), Building 2 (Power Plant), and Building 11 (Spinal Cord Injury Building) are known to currently have asbestos containing materials present.
Based on Millennium’s understanding of the client’s needs, the following scope of services was conducted:
1. Visual reconnaissance of the site to document homogeneous areas, locate known/suspect ACM and assess conditions.
2. Asbestos reinspection and assessment of buildings 1, 2, 9, 11, 12, 14, 16, 17, 18, 20, 23, 24, 25, 27, and 28 at the Jennifer Moreno Department of Veterans Affairs Medical Center in accordance with the listed criteria in California Occupational Safety and Health Administration (Cal-OSHA) standard 8 California Code of Regulations (CCR) 1529, OSHA standard 29 Code of Federal Regulations (CFR) 1926.1101, Environmental Protection Agency (EPA) AHERA standard 40 CFR Part 763 and EPA standard.
3. Preparation of a written report detailing the survey information, including locations, conditions of accessible ACMS and recommendations for the future.
Millennium conducted the Asbestos 6-Month Reinspection March 2024 in general accordance with industry standards and procedures in existence at the time of the project for visual inspection of known ACMs.
The conclusions and recommendations presented in this report are based on the applicable standards of our profession at the time this report was prepared. Copies of this report are furnished to provide the factual data that was gathered and summarized in the report.
2.0 LIMITING CONDITIONS
The analyses and recommendations submitted in this report are based on the analytical data obtained from the Veteran Medical Center of San Diego specific and discrete sampling locations from representative building components were done by others. However, the nature and extent of variations between the sampling locations may not become evident until planned renovation procedures commence. If potential variations are identified during renovation activities, it will be necessary to conduct additional bulk sampling and assessments.
While Millennium made every effort to accurately quantify asbestos containing materials, all quantities are estimates, both visibility and access are inherently constrained by the nature of patient and research level spaces. Additional limiting factors include lack of access to current architectural as-builts and hidden
Millennium Project No. 22004.2200 -8- 22004.2200 conditions (whereby visual or physical access was limited). Exact quantities should be verified by the contractor in the field. All ACM quantification is based on the ability of the inspector to clearly view the substrate or building component being quantified. Many areas exist within the scope of this project where visibility and access were low.
This report has been prepared for the exclusive use of the Department of Veterans Affairs - San Diego for specific application to the ACM materials survey performed on the property. This report may not be copied (except by our client) without the written permission of the Department of Veterans Affairs-San Diego. No other representation, expressed or implied, is made.
3.0 SITE DESCRIPTION
The project site consists of fifteen buildings denoted as buildings 1, 2, 9, 11, 12, 14, 16, 17, 18, 20, 23, 24, 25, 27, and 28. The main hospital facility Building 1 which is a seven story, 855,000 square foot Medical Center that was built between 1969-1972, located at: 3350 La Jolla Village Dr, San Diego, California, 92161.
Building 1 is known contain the vast majority of known ACM. The in-scope building, building 1, is comprised of six main floors, a basement, with an interstitial space above each floor, a penthouse level (7th Floor) and elevator room on top of the penthouse.
4.0 ACM MATERIAL SURVEY
The initial asbestos survey was performed between June 2023 to August 2023 Kevin Cullen (CAC #10-6866), Dylan Ridgel (CAC #22-7067) and Cornelius Harris (CSST #22-7041). The 6-Month Reinspection was performed by Cornelius Harris in March 2024.
4.1.1 ACM Survey Overview – Building 1 Patient Level
As part of the survey methodology, using the initial 2023 Asbestos survey report Millennium performed a walk-through of Building 1. March 2024 material conditions were compared to the initial 2023 survey to determine what if any changes to ACM’s has occurred.
Most accessible areas of the interstitial/patient levels, research level including the Penthouse were accessed (see summary table for specific non accessible rooms.) Bulk sampling was not performed during the Asbestos 6-Month Reinspection. Below grade appurtenances and areas outside of the scope were not evaluated.
Building One was reinspected for known asbestos-containing surfacing materials, asbestos-containing miscellaneous friable materials, suspect asbestos-containing Category I non-friable materials, and suspect asbestos-containing Category II non-friable materials. Friable materials are defined as those materials, when dry, that can be crumbled or reduced to powder by hand pressure. Category I non-friable materials are defined as packing, gaskets, asphalt roofing materials and resilient flooring materials and associated mastics in which the asbestos fibers are bound within a resinous matrix. Category II non-friable materials are defined as other non-friable materials such as asbestos cement in which the asbestos fibers are bound within a cement-like matrix.
The 6-Month Asbestos Reinspection was a visual assessment of known ACM materials; based upon Millennium’s 2023 3- Year Asbestos Survey.

4.1.3 ACM Survey Overview – Building 1 Interstitial Spaces
As part of the survey methodology, a preliminary walk-through of the subject property’s building was performed to familiarize the inspectors with the structures and to identify suspect ACM.
Most accessible areas within the interstitial were accessed except for mechanical rooms KX-8 A2025 / KX-7 A2024 / KX-6 A2023 / KX-5 A2022; below catwalks; and interior of wall cavities that are open to the interstitial and vertical shaft areas. Bulk sampling was performed in select areas deemed to be representative. Below grade appurtenances and areas outside of the scope were not evaluated.
During the walk-through, Building One was assessed for suspect asbestos-containing surfacing materials, suspect asbestos-containing miscellaneous friable materials, suspect asbestos-containing Category I non-friable materials, and suspect asbestos-containing Category II non-friable materials.
4.1.4 ACM Re-Inspection Findings - Building 1 Interstitial Spaces
Tables 9-16 provide an inventory of asbestos containing materials from the interstitial spaces of Building 1.

* Sprayed on fireproofing quantities are estimates based on field observations only.
* Brown HVAC duct mastic quantities are estimates based on field observations only.
Due to lack of access to architectural plans and the poor general accessibility within the interstitials, exact quantities should be verified by the contractor in the field.
The remaining materials’ quantities are estimates with variation possible at +/- 15%. Exact quantity should be verified by contractor in the field.
