About this file

This is a three-year asbestos inspection report for the Jennifer Moreno Department of Veterans Affairs Medical Center in San Diego, California, dated December 6, 2023. The report documents a comprehensive hazardous materials survey of 15 buildings (1, 2, 9, 11, 12, 14, 16, 17, 18, 20, 23, 24, 25, 27, and 28) conducted between June and September 2023 by Millennium Consulting Associates. The primary focus was Building 1, a seven-story, 855,000 square foot medical center constructed between 1969-1972, with additional attention to Building 11. Inspectors Kevin Cullen (CAC #10-6866) and Dylan Ridgel (CAC #22-7067) conducted field reconnaissance and collected 880 samples comprising 1,189 discrete layers of suspect asbestos-containing materials throughout accessible areas, including patient levels, interstitial spaces, and the penthouse level.

The report identifies significant asbestos-containing material (ACM) throughout Building 1, including dark grey spray-applied fireproofing at structural steel and concrete decks (approximately 1,400,000 square feet containing 4-30% asbestos), damaged fireproofing debris in interstitial levels (100,000 square feet containing 6-20% asbestos), thermal system insulation at 13,500 linear feet containing 2-50% asbestos, brown and red duct mastic totaling approximately 13,700 square feet with 2-15% asbestos content, drywall/joint compound in various conditions, and twelve-inch resilient floor tiles with black mastic throughout all patient levels (112,778 square feet confirmed and 62,265 square feet presumed below carpeting). Building 11 contains twelve-inch floor tile with black mastic containing 2% chrysotile asbestos. The report provides detailed recommendations for ACM management, including compliance with Cal/OSHA standards, regular inspection schedules for high-traffic areas, contractor requirements limiting attachments to fireproofing, development of above-ceiling access procedures, task-specific cleanup plans for damaged fireproofing, and careful maintenance protocols for known ACM flooring.

EXECUTIVE SUMMARY

Millennium Consulting Associates (Millennium) was retained by the Department of Veterans Affairs to conduct an Asbestos Hazardous Material Survey of fifteen buildings comprising the Jennifer Moreno Veterans Affairs Medical Center. The survey included buildings 1, 2, 9, 11, 12, 14, 16, 17, 18, 20, 23, 24, 25, 27, and 28. The Jennifer Moreno Medical Center is located at 3350 La Jolla Village Dr, San Diego, California 92161. The Hazardous Materials survey was conducted from June 2023 through September 2023. The original Veterans Affairs Hospital of San Diego (Building 1) a seven story, 855,000 square foot Medical Center that was built between 1969-1972.

Millennium’s services included the performance of an Asbestos Hazardous Material Survey, Assessment, and report. Millennium performed field reconnaissance and sample collection from June 2023 to September 2023. Millennium conducted a walk-through of the project site to identify and collect information regarding asbestos containing materials included in the scope of work. Millennium used the available information to create a sampling strategy representing suspect materials located throughout the aforementioned areas.

For the ACM survey, Millennium collected eight-hundred-and-eighty (898) samples, and one-thousand-one hundred-eighty-nine (1,209) discrete layers of suspect Asbestos Containing Materials (ACM) throughout the areas assessed in Building 1.

Prior to maintenance, renovation, or demolition, if affected, the following materials should be removed and disposed of as asbestos containing materials by a licensed and a qualified asbestos contractor in accordance with all federal, state, and local laws, ordinances, and regulations.

Building 1

• Dark grey spray applied fireproofing at the structural steel components and overspray at the concrete deck throughout all levels of the interstitial and penthouse contains between 4% and 30% asbestos; the estimated quantity is 1,400,000 square feet. The spray applied fireproofing/overspray was observed at: the upper sections of the wall assemblies at the concrete deck, HVAC ductwork, the yellow fiber glass panel boards at the concrete deck, shafts, and columns throughout the interstitial. The spray applied fireproofing within the interstitial was observed to sporadically have white encapsulation and/ or painted red, green, yellow, and blue, concealing the dark grey original fireproofing.

Millennium Project No. 22004.2200 -5- 22004.2200

• The spray applied fireproofing was significantly damaged at multiple locations throughout all interstitials including at the top side of the T- Bar ceiling system at each interstitial level.

Fireproofing debris at the interstitial levels contain between 6% and 20% asbestos; the estimated impacted area is 100,000-ft2. The fireproofing debris locations include: vertical shaft and column enclosures at the topside of HVAC ductwork; above and below interstitial catwalks at the topside of T- bar grid ceiling tiles; commingling with fiber glass insulation at the topside of the T-bar ceiling; concrete floors of mechanical/electrical rooms; and multiple inaccessible areas at the patient level.

• Thermal System Insulation (TSI) at all interstitial levels and penthouse contains between 2% and 50% asbestos; the estimated quantity is 13,500-linear feet. Thermal System Insulation components include:

hard packed elbows; tees, fittings, internal/external pipe hangers; mechanically fastened cloth blankets; asbestos board at ductwork; gaskets, grey felt and adhesive; canvas wrapped hard packed pipe runs; brown canvas wrapped TSI; and white TSI fabric wrap.

• Brown duct mastic at all interstitial levels and penthouse contains between 2% and 15% asbestos;

the estimated quantity is 12,500 square feet. The brown duct mastic was observed on HVAC components including rectangular ductwork; round ductwork, insulated duct work; interior of ductwork; metal boots at T- bar ceiling; and HVAC equipment. Note: quantities only account for visible duct mastic. Duct mastic below the insulation and at the interior of the ductwork should be considered a hidden condition.

• Red duct mastic at all interstitial levels contains between 4% asbestos; the estimated quantity is 1,200 square feet. The red duct mastic was observed on HVAC components including rectangular ductwork, round ductwork, metal boots and HVAC equipment.

• Original finished drywall/joint compound and drywall/joint compound with yellow tape contains 2% asbestos in the joint compound. The estimated quantity is 11,900 square feet. The original finished drywall/joint compound and tape was observed at the first-floor interstitial RM A1011A and penthouse.

• Original unfinished drywall/joint compound and tape contains 2% asbestos. The estimated quantity is 5,000 square feet. The original unfinished drywall/joint compound and tape was observed at all interstitial levels throughout. Note: Unfinished drywall without joint compound and distinguishable newer application drywall and joint compound is negative for asbestos.

• Several types of twelve-inch resilient floor tile including black floor mastic throughout all patient levels including the sixth-floor research level contains asbestos; the estimated quantity is 112,778 square feet. The twelve-inch resilient floor tile and black floor mastic was observed at: Corridors, offices, special use rooms, common areas, labs, patient rooms, restrooms, closets, and storage rooms. For specific material component and locations refer to: Summary of patient level remaining asbestos containing materials can be found in Tables 1, 3, 5, 7, 9, 11, 13 and 15.

Millennium Project No. 22004.2200 -6- 22004.2200

• The twelve-inch resilient floor tile including black floor mastic throughout all patient levels including the sixth-floor research level, below carpeting is presumed to contain asbestos; the estimated quantity is 62,265 square feet. The presumed twelve-inch resilient floor tile including black floor mastic below the carpeting is located at: Corridors, offices, storage rooms, and common areas.

• The presumed transite fume hoods and laboratory countertop systems contain asbestos; The presumed transite fume hoods and countertop systems are located through the research laboratory rooms of the sixth floor.

Building 11

• Black mastic underneath several types twelve-inch resilient floor tile including; brown speckled, beige speckled, blue speckled, gray speckled and various other colors used to repair/replace damaged floor tiles. The mastic contains 2% chrysotile asbestos and is presumed underneath all twelve-inch floor tile. Many areas of the first floor have damaged floor tile with exposed black/gray mastic exposed. The first floor of Building 11 has twelve-inch floor tile throughout.

The second floor has been remodeled and all twelve-inch tile has been removed or encapsulated by new flooring systems.

Asbestos-Containing Material (ACM) regulations are published by the U.S. Environmental Protection Agency (EPA) and the State of California, Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA). A material is considered by the EPA to be ACM if at least one sample collected (by homogeneous group) is analyzed and asbestos is detected in an amount greater than one percent (>1%).

Both Federal OSHA and Cal/OSHA consider a material to be ACM if it is found to contain greater than one percent (>1%) asbestos. In addition, Cal/OSHA defines Asbestos-Containing Construction Material (ACCM) as a material containing greater than one-tenth of one percent (>0.1%) asbestos. Employers performing construction activities where ACCM is present must comply with all requirements of registration by Cal/OSHA for asbestos-related work and all applicable provisions of the California Code of Regulations, Title 8, Section 1529.

Summary tables containing sampling results, material descriptions, material locations, EPA categories and damage assessment for the entire ACM survey may be found in subsection 4.2.

Recommendations Based on the data collected and referenced herein, Millennium recommends the following course of actions:

Millennium Project No. 22004.2200 -7- 22004.2200

1. All Regulated Asbestos-containing Materials (RACM) that will be disturbed or affected by any future work shall be done in compliance with the asbestos National Emissions Standards for Hazardous Air Pollutants (NESHAP), and Cal-OSHA Asbestos in the Construction Industry Standard, 8 California Code of Regulations (CCR) 1529. Additionally, all Category I and Category II non-friable asbestos-containing materials that may become friable as a result of renovation/demolition work and that may be affected by the planned renovation shall be removed prior to renovation/demolition (or other activities resulting in the disturbance of these types of ACM) of the subject building in compliance with the San Diego Air Pollution Control District Rule 1206, National Emission Standards for Hazardous Air Pollutants (NESHAPS), and Cal- OSHA Asbestos in the Construction Industry Standard, 8 CCR 1529.

2. Materials containing any detectable concentration of asbestos impacted by a planned renovation shall be handled in accordance with the Cal/OSHA Asbestos in the Construction Industry Standard, 8 CCR 1529 and the Federal OSHA 1926.1101.

3. All assumed asbestos-containing materials referenced herein shall be treated as asbestos-containing materials and managed accordingly, or – shall be sampled and analyzed for asbestos content prior to renovation.

4. A regular inspection schedule and assessment of all patient levels including the sixth-floor research level and Penthouse should be performed, directly related to the management of ACM flooring, wall systems and TSI especially in high traffic areas with potential for damage.

5. Implement contract requirements for contractors to reinforce existing regulations. This includes during work activities within the patient levels and research level that require the cleanup of any asbestos debris within the work areas or paths of travel. Under Cal/OSHA and Federal OSHA, asbestos hazards resulting from work activities shall be abated by the contractor who created or controls the source of the contamination (see 1926.1101(d)(2) and 1529(d)(2)).

6. Implement contract requirements to limit any attachment to the asbestos-containing fireproofing for any future utilities or equipment installation (e.g., ethernet cables, patch cables, electrical conduit, HVAC strapping, ladders, toe boards, scaffolding etc.). Also, ensure that contractors and other workers are not drilling, fastening, or coring into the asbestos-containing fireproofing overspray at the concrete deck during work.

7. An above ceiling access procedure should be developed for maintenance/engineering staff and for contractors when ceiling tiles are moved to access the above ceiling space.

8. A task specific plan for the cleanup of ACM debris throughout the interstitial levels including the repair and encapsulation of damaged fireproofing at structural members, particularly at beams that are within a close proximity to the catwalks and other paths of travel.

9. Special care should be taken when buffing, waxing and or cleaning flooring that is a known

ACM.

Millennium Project No. 22004.2200 -8- 22004.2200

In summary, building owners are responsible for how asbestos-containing materials, presumed asbestos-containing materials (PACM), and assumed asbestos-containing materials are handled at and within their buildings. This includes tenants, maintenance and custodial workers, contractors, and others. Building owners must also verify asbestos remaining at or within their buildings is properly managed and maintained in a nonhazardous fashion. This includes notifications to various parties and labeling requirements within OSHA regulations.

Three Year ACM Inspection

Millennium Project No. 22004.2200 -9- 22004.2200

1.0 INTRODUCTION

Of the fifteen buildings included in the inspections only Building 1 (Main Hospital) and Building 2 (Spinal Cord Injury Building) are known to currently have present asbestos containing materials.

Based on Millennium’s understanding of the client’s needs, the following scope of services was conducted:

1. Visual reconnaissance of the site to document homogeneous areas, locate known/suspect ACM and assess conditions.

2. Asbestos survey and assessment of the patient levels and interstitial spaces including the research level and Penthouse in accordance with the listed criteria in California Occupational Safety and Health Administration (Cal-OSHA) standard 8 California Code of Regulations (CCR) 1529, OSHA standard 29 Code of Federal Regulations (CFR) 1926.1101, Environmental Protection Agency (EPA) AHERA standard 40 CFR Part 763 and EPA standard.

3. Preparation of a written report detailing the survey information, including locations, conditions of accessible ACMS and recommendations for the future.

Millennium conducted the Asbestos Hazardous Material Inspection between the months of June 2023 and September 2023 in general accordance with industry standards and procedures in existence at the time of the project for visual inspection of known ACMs.

The conclusions and recommendations presented in this report are based on the applicable standards of our profession at the time this report was prepared. Copies of this report are furnished to provide the factual data that was gathered and summarized in the report.

2.0 LIMITING CONDITIONS

The analyses and recommendations submitted in this report are based on the analytical data obtained from the Veteran Medical Center of San Diego specific and discrete sampling locations from representative building components were done by others. However, the nature and extent of variations between the sampling locations may not become evident until planned renovation procedures commence. If potential variations are identified during renovation activities, it will be necessary to conduct additional bulk sampling and assessments.

While Millennium made every effort to accurately quantify asbestos containing materials, all quantities are estimates, both visibility and access are inherently constrained by the nature of patient and research level

Millennium Project No. 22004.2200 -10- 22004.2200 spaces. Additional limiting factors include lack of access to current architectural as-builts and hidden conditions (whereby visual or physical access was limited). Exact quantities should be verified by the contractor in the field. All ACM quantification is based on the ability of the inspector to clearly view the substrate or building component being quantified. Many areas exist within the scope of this project where visibility and access were low.

This report has been prepared for the exclusive use of the Department of Veterans Affairs - San Diego for specific application to the ACM materials survey performed on the property. This report may not be copied (except by our client) without the written permission of the Department of Veterans Affairs-San Diego. No other representation, expressed or implied, is made.

3.0 SITE DESCRIPTION

The project site consists of fifteen buildings denoted as buildings 1, 2, 9, 11, 12, 14, 16, 17, 18, 20, 23, 24, 25, 27, and 28. The main hospital facility Building 1 which is a seven story, 855,000 square foot Medical Center that was built between 1969-1972, located at: 3350 La Jolla Village Dr, San Diego, California, 92161.

Building 1 is known contain the vast majority of known ACM. The in-scope building, building 1, is comprised of six main floors, a basement, with an interstitial space above each floor, a penthouse level (7th Floor) and elevator room on top of the penthouse.

4.0 ACM MATERIAL SURVEY

Kevin Cullen (CAC #10-6866), Dylan Ridgel (CAC #22-7067) performed this asbestos survey and assessment from June 2023 to September 2023.

4.1.1 ACM Survey Overview – Building 1 Patient Level

As part of the survey methodology, a preliminary walk-through of the subject property’s building was performed to familiarize the inspectors with the structures and to identify suspect ACM.

Most accessible areas at the patient levels and research level including the Penthouse were accessed (see summary table for specific non accessible rooms.) Bulk sampling was not performed during the Asbestos Hazardous Materials Inspection. Below grade appurtenances and areas outside of the scope were not evaluated.

During the walk-through, Building One was assessed for suspect asbestos-containing surfacing materials, suspect asbestos-containing miscellaneous friable materials, suspect asbestos-containing Category I non-friable materials, and suspect asbestos-containing Category II non-friable materials. Friable materials are defined as those materials, when dry, that can be crumbled or reduced to powder by hand pressure. Category I non-friable materials are defined as packing, gaskets, asphalt roofing materials and resilient flooring materials and associated mastics in which the asbestos fibers are bound within a resinous matrix. Category II non-friable materials are defined as other non-friable materials such as asbestos cement in which the asbestos fibers are bound within a cement-like matrix.

During the walk-through, the homogeneous sample groups (HSGs) were identified and based upon historical analytical data provided to Millennium by the Veterans Medical Center of San Diego.

Millennium Project No. 22004.2200 -11- 22004.2200

From June 2023 to September 2023, Millennium did not collect samples. The Asbestos Hazardous Material Inspection was a visual assessment of known ACM materials; based upon historical analytical data and previous inspection reports. At the direction of the VA the Encorp 2015 ACM Survey Report was utilized as a foundation for Millennium’s Building 1 survey.
