About this file
This is an affidavit form required by Florida Statutes Section 287.138 for entities contracting with Hillsborough County Public Schools (HCPS) and the GOVMVMT Purchasing Cooperative regarding substitute teacher services. The affidavit must be completed and executed by an authorized representative of any proposer or contractor submitting a bid, proposal, or quote for the master agreement opportunity. No bid, proposal, or other response will be accepted without submission of this completed and executed affidavit. The form establishes that the proposer/contractor does not meet any of the criteria that would prohibit contracting with the governmental entity under Florida law.
The affidavit requires attestation that the contracting entity is not owned by, does not have a controlling interest held by, and is not organized under the laws of or principally based in a foreign country of concern, as defined in the statute. Foreign countries of concern identified include the People's Republic of China, the Russian Federation, the Islamic Republic of Iran, the Democratic People's Republic of Korea, the Republic of Cuba, the Venezuelan regime of Nicolas Maduro, and the Syrian Arab Republic. The authorized representative must declare under penalties of perjury that the facts stated in the affidavit are true. "Controlling interest" is defined as possession of the power to direct management or policies, including direct or indirect rights to vote 25 percent or more of voting interests or entitlement to 25 percent or more of profits.
