About this file

This is a Certificate of Partial Payment document related to Clay County, Florida's Indigent Decedent Services contract opportunity, which seeks a qualified funeral home to administer the county's Unclaimed Decedent Disposition Program (UDDP) and provide related services. The certificate serves as a payment authorization form for vendors performing work under the contract, requiring vendors to certify that all work has been performed and materials supplied in accordance with contract terms and conditions. The document establishes payment submission and processing procedures as part of the broader solicitation process, with the bidding period commencing from the publication date and extending until a final bid decision is reached, including any applicable 72-hour protest periods.

The certificate of partial payment form outlines critical payment flow requirements, mandating that prime contractors remit payments to subcontractors and suppliers within 10 days of receiving payment, with subcontractors further obligated to remit payments to their respective subcontractors and suppliers within 7 days. The vendor must certify that all subcontractors and suppliers with contract interests have received their pro rata shares from previous progress payments, less any retainage withheld pursuant to subcontractor agreements. The form includes designated fields for payment number, total amount due, date, vendor identification, and authorized signature, establishing accountability and compliance mechanisms for the payment distribution chain throughout the contract performance period.
