About this file
The document is a Bid Submittal Form for a Job Order Contract (JOC) issued by Pinellas County, Florida, specifically for the Jail Elevator Modernization project at the Central Intake Building. The bid form outlines a comprehensive pricing structure for work in various areas and working conditions, including non-secure and secure areas during normal and non-normal working hours. The contract involves modernization of three elevators in a seven-story building, with one simplex elevator and one duplex elevator (two cabs in a single shaft), located at 14400 49th Street N., Clearwater, FL.
The bid pricing mechanism uses an Adjustment Factor system where bidders propose a multiplier to standard Construction Task Catalog® unit prices. The bid evaluation will use weighted percentages across five categories: Non-Secure Areas (Normal Hours at 60%), Non-Secure Areas (Other Hours at 10%), Secure Areas (Normal Hours at 10%), Secure Areas (Other Hours at 10%), and Non Pre-priced Tasks (10%). Bidders must submit uniform Adjustment Factors to four decimal places, with specific requirements such as ensuring that non-normal working hours factors are greater than or equal to normal working hours factors. The County reserves the right to correct arithmetic errors and uses the submitted factors to calculate an Award Criteria Figure for bid determination.

We, the undersigned, hereby declare that no person or persons, firm or corporation, other than the undersigned, are interested in this proposal, as principals, and that this Proposal is made without collusion with any person, firm or corporation, and we have carefully and to our full satisfaction examined the Contract Documents, and that we have made a full examination of the location of the proposed work and the source of supply of materials, and we hereby agree to furnish all necessary labor, equipment, and materials, fully understanding that this is an indefinite quantity Contract, and that we will fully complete all necessary work in accordance with the Contract Documents, within the time limit specified in each Job Order for the unit prices set forth in the Construction Task Catalog®, multiplied by the Adjustment Factors inserted below.
