About this file

This file is an Installed Equipment Contract template and example for documenting equipment and materials installed during a federal construction project. The template is designed to track equipment installations for Building 514 Subfloor Removal & Epoxy Coating project at Naval Undersea Warfare Center (NUWC) Division, Keyport under Contract N0025326Q0005. Contractors must complete the spreadsheet with detailed information for each asset installed, including building and room numbers, equipment descriptions, quantities, unit of measure, total installed costs (labor and material for both removal and installation), supplier contact information, manufacturer details (name, model number, serial number), warranty dates, and the asset number of any equipment removed. Each item with a unit of issue of "EACH" requires a separate line item.

The example provided demonstrates the required documentation format using two Breathing Air Filters and carpet installation. The Breathing Air Filters, supplied by Atlas Copco (Model FTX345BU) at $2,400.00 each, include manufacturer warranty expiration of 3/12/21 and installation date of 3/12/17. The carpet installation for rooms 116, 117, and 118 totaling 300 square feet was supplied by Mohawk through a Carpets vendor at $4,200.00. Contractors must submit completed spreadsheets to the government, which will assign new asset numbers during the FMS intake process. The template emphasizes that costing data must encompass all labor and material expenses associated with both removal of existing assets and installation of new equipment, and that digital submission via email to designated Navy contacts is required rather than paper copies or company quote forms.
