About this file
This is a Notice to Proceed Submittals checklist used by SLAC National Accelerator Laboratory's Supply Chain Management division for subcontractor compliance prior to project commencement.
The checklist establishes eight mandatory submittal requirements for subcontractors. Performance and Payment Bonds with penal sums equal to the Fixed Firm Price award are required for subcontracts valued at $35,000 and over. Approved insurance certificates must be provided by all subcontractors. For subcontracts of $25,000 and over involving design work, subcontractors must obtain Professional Errors and Omissions Insurance with minimum coverage of $1,000,000 per claim. Subcontracts valued at $50,000 and over require Builder's All Risk Insurance covering physical damage to work performed, with coverage at full value of work at time of loss or damage. Additional requirements include submission of SF1413 Statement and Acknowledgment forms for all known sub-subcontractors, copies of the subcontractor's licenses and permits and those of sub-subcontractors as needed, Issue for Construction Set (IFC) drawings and specifications, and a project schedule in the format specified by the SLAC Project Manager. This template is designed to ensure compliance and risk mitigation prior to issuing notice to proceed on construction or service delivery subcontracts.
