About this file
This is a Reference Information Sheet template used for proposal submissions in response to the Ground Maintenance and Tree Assessment solicitation for USAG Wiesbaden (Solicitation Number W912CM26QA014), issued by the Department of the Army Materiel Command Army Contracting Command. The template requires bidders to provide one reference sheet for each relevant past performance example included in their proposal.
The Reference Information Sheet requires contractors to document comprehensive details about previous contract experience, including the contracting organization's name and address, contract number, award and completion dates, total contract value, and points of contact for both technical and contracting personnel. Bidders must describe the scope and complexity of work performed, note any performance issues or customer conflicts, and disclose any litigation history. The template also requires contractors to indicate the current contract status across multiple categories, such as work continuing on schedule, work completed with routine administrative actions pending, or various terminated or disputed statuses. References must demonstrate relevant experience within the five years prior to the solicitation issue date. This form is applicable to the five-year Indefinite Delivery Indefinite Quantity (IDIQ) Firm Fixed Price contract for ground maintenance and tree assessment services in Germany, with the previous contract valued at approximately $4.8 million.
