About this file
This is a final construction documents specification file for the Whiteriver Hospital replacement project located on the Fort Apache Indian Reservation in Arizona, being constructed for the Indian Health Service (IHS). The document is Volume 3 of the specifications, dated January 23, 2026, prepared by SmithGroup and multiple engineering firms including PK Associates, Dibble & Associates, and Morrison-Maierle, covering divisions 21 through 46 of the construction specifications.
The specifications detail comprehensive requirements for fire suppression systems, plumbing systems, and wastewater treatment plant (WWTP) equipment and installation. Key requirements include fire sprinkler and standpipe systems designed per NFPA 13 and 14 standards with wet-pipe, dry-pipe nitrogen-filled, and preaction system options; fire pump and pressure-maintenance pump installations with controllers and monitoring equipment; domestic water piping using copper tube and fittings with specific joining methods; plumbing equipment insulation and piping insulation requirements for various applications; and detailed WWTP specifications covering membrane bioreactor systems, sludge dewatering, screening, and aeration equipment. The specifications mandate commissioning of systems per IHS requirements, field quality control testing and inspections, operator training, and extensive documentation including as-built record drawings. The 407,000-square-foot hospital project encompasses primary care, emergency services, specialty care, acute care beds, diagnostic imaging, pharmacy, laboratory, and various support services, with the new facility designed to serve a projected user population of 19,088 by 2030 in the White Mountain Apache area.

SECTION 01 91 13 – GENERAL COMMISSIONING REQUIREMENTS.
1.4 SUBMITTALS
A. The commissioning process requires review of selected Submittals. The CxA will provide a list of Submittals earmarked for CxA review. This list will be reviewed and approved by IHS prior to sharing with the contractor. Refer to SECTION 01 33 00 – SUBMITTAL PROCEDURES for further details.
B. The commissioning process requires Cx Submittal review simultaneous to A/E Submittal review. Specific submittal requirements related to the Commissioning Process are specified in SECTION 01 91 13 – GENERAL COMMISSIONING
REQUIREMENTS.
PART 2 - PRODUCTS (Not Used)
PART 3 - EXECUTION
3.1 START-UP AND CONSTRUCTION INSPECTIONS
A. General. Commissioning of the Building Fire Suppression Systems will require inspection of individual elements of the Fire Suppression System construction throughout the construction period. The Contractor shall coordinate with the CxA in accordance with SECTION 01 91 13 – GENERAL COMMISSIONING REQUIREMENTS and the Commissioning Plan to schedule inspections as required to support the commissioning process.
FIRE SUPPRESSION SYSTEM COMMISSIONING 210800 - 2
3.2 PRE-FUNCTIONAL CHECKLISTS
A. General. Pre-functional Checklists are developed and completed for all major equipment and systems being commissioned. The checklist captures equipment nameplate and characteristics data, confirming the as-built status of the equipment or system. These checklists also ensure that the systems are complete and operational, so that the functional performance testing can be scheduled. The checklists are drafted by the CxA and completed (filled out) by the installing Contractor.
B. Review. Upon notification of checklist completion, the CxA and IHS will review the Pre- Functional Checklists for accuracy. If the CxA or IHS determine that the information provided on a checklist is not accurate, the marked-up checklist will be returned to the Contractor for correction and resubmission. An item be added to the Commissioning Issue Log (CIL) to track the deficiency.
If the CxA or IHS determine that a significant number of completed checklists for similar equipment are not accurate, all the checklists for the equipment type will be returned to the Contractor for correction and resubmission.
Refer to SECTION 01 91 13 – GENERAL COMMISSIONING REQUIREMENTS
3.3 FUNCTIONAL PERFORMANCE TESTING
A. Objectives and Scope. The objective of functional performance testing is to demonstrate that each system is operating according to the documented design intent and Contract Documents. In general, each system should be operated through all modes of operation (seasonal, occupied, unoccupied, warm-up, cool-down, part-and full-load) where there is a specified system response. Verifying each sequence in the sequences of operation is required. Proper responses to such modes and conditions as power failure, freeze condition, no flow, equipment failure, etc. shall also be tested.
B. Development of Test Procedures. Before test procedures are written, the CxA shall obtain all requested documentation and a current list of change orders affecting equipment or systems, including an updated points list, program code, control sequences and parameters. The CxA shall develop specific test procedures and forms to verify and document proper operation of each piece of equipment and system. Prior to execution, the CxA shall provide a copy of the test procedures to the Contractor who shall review the tests for feasibility, safety, equipment and warranty protection. The CxA shall review contracted or factory testing which the CxA is not responsible to oversee and shall determine what further testing may be required to comply with the Contract Documents. Redundancy of testing shall be minimized.
C. Test Methods:
1. Functional performance testing and verification may be achieved by manual testing (persons manipulate the equipment and observe performance) or by monitoring the performance and analyzing the results using the control system’s trend log capabilities or by stand-alone data loggers. The CxA will determine which method is most appropriate.
2. Setup. Each function and test shall be performed under conditions that simulate actual conditions as close as is practically possible. The Contractor executing the test shall provide all necessary materials, system modifications, etc. to produce the necessary flows, pressures, temperatures, FIRE SUPPRESSION SYSTEM COMMISSIONING 210800 - 3 etc. necessary to execute the test according to the specified conditions. At completion of the test, the Contractor shall return all affected building equipment and systems, due to these temporary modifications, to their pre-test condition.
3. Sampling. Multiple identical pieces of non-life-safety or non-critical equipment may be functionally tested using a sampling strategy. The sampling strategy will be developed by the CxA and approved by the Owner’s Representative.
For details on Sampling, refer to the Commissioning Plan. If, after three attempts at testing the specified sample percentage, failures are still present, then all remaining units are tested at the Contractor’s expense.
D. Coordination and Scheduling. The Contractor shall provide sufficient notice to the CxA regarding their completion schedule for the pre-functional checklists and startup of all equipment and systems. The CxA will schedule functional tests through the Owner’s Representative. The CxA shall direct, witness and document the functional testing of all equipment and systems. The Contractor shall execute the tests.
E. Problem Solving. The CxA will recommend solutions to problems found; however, the burden of responsibility to solve, correct, and retest problems lie with the Contractor and their subcontractors.
3.4 O&M PERSONNEL TRAINING REQUIREMENTS
A. Training Plans: Contractor shall develop an instructional program that includes individual training modules for each system, subsystem, and equipment as specified in SECTION
01 79 00 – DEMONSTRATION AND TRAINING.
B. The minimum training requirements for Fire Suppression Systems are specified in technical sections.
C. The CxA shall review and provide comment on the training plans developed by the contractor for alignment with the requirements of the OPR and BOD.
