About this file
This document is a score sheet for evaluating proposals submitted in response to a Request for Proposal (RFP) issued by the State of Utah for patient/client support services for wheelchairs, lifts, ramps, home modifications, and accessories. The RFP, solicitation # AA25-15, is seeking to add more vendors to an existing state cooperative contract for these services.
The proposal evaluation criteria includes a technical component worth 80% of the total score, focusing on the vendor's services, experience, technical certifications, training, knowledge, and customer service processes. There is also a cost component worth 20% of the total score, which considers the vendor's discount percentage on catalog items and average hourly installation rates. Vendors must meet a minimum technical score threshold of 60% to be considered. The contract term and any renewal options are not specified.

| 1 | Category 1: Services and Experience - Please list and describe in detail the types of services offered and the experience for each type. e.g. if you provide vehicle modifications and home lifts, please describe the depth of your knowledge within these categories and your historical experience providing these services. Please do not simply state you have been providing services for 15 years. Vendor must elaborate on their specific experience and how it will provide VALUE to the State of Utah. The evaluators will evaluate solely on the quality of your experience for the categories you can provide. For example, if you specialize in vehicle modifications only, you will not be penalized for not being able to provide in home modifications. The team will look solely at the quality and experience in that designated category. | Prerequisite | 60 | 43.8% | 350.0 | 0.0 | |

| Category 1: 100 points are available based upon what discount percentage you will offer on all items/goods available within your catalog. 0 - 3% Discount = 20 Points, 4 - 6% Discount = 40 Points, 7 - 9% Discount = 60 Points, 10 - 13% Discount = 80 Points, and 14+ Discount = 100 Points | |||||||
| Category 2: 100 points are also available on your average hourly rate to install/complete a project. Most products will require installation and/or modification within an existing structure or vehicle. The lowest hourly rate will receive 100 points. All others will receive a portion of points depending on the ratio of their average installation cost to the lowest. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what ratio higher their Price is than the Lowest Price.
