About this file

This document appears to be Attachment C Cost Sheet for a state cooperative contract opportunity from the State of Utah for Patient Mobility Home and Vehicle Modifications. The state is seeking to add more vendors to this existing contract, which includes product categories such as vehicle lifts, home lifts, stair lifts, ceiling lifts, ramps, shower/bath products, and other adaptive equipment. Vendors must provide pricing discounts off MSRP as well as hourly installation rates, with cost points awarded based on the level of discount and installation rate offered. Vendors that do not provide installation services will receive double the cost points for their discount percentage. The contract will award a total of 200 cost points, with 100 points for discount percentage and 100 points for installation rate.

The pricing structure requires vendors to complete the yellow-shaded cells on the cost sheet, with the lowest hourly installation rate receiving the full 100 installation rate points. Discount percentage points range from 20 points for 0-3% discounts up to 100 points for 14%+ discounts. This is an existing cooperative contract, so there may be incumbent vendors already providing services under the agreement.
