About this file
This document is a scoring sheet for the evaluation of proposals submitted in response to Solicitation #AA25-15, a Request for Proposals (RFP) from the State of Utah for Patient/Client Support for Wheelchairs, Lifts, Ramps, Modifications, and Accessories. The evaluation criteria include minimum mandatory requirements such as having necessary licenses, submitting a technical response, cost sheet, and product catalog. The scoreable technical criteria are divided into three categories: Services and Experience (60% of technical points), Technical Certification, Training, and Knowledge (40% of technical points), and Customer Service (25% of technical points). Proposals must achieve a minimum of 60% of the total technical points to be considered. Cost points make up 20% of the total evaluation, with discounts on catalog items and hourly installation rates factoring into the scoring. This is an existing state cooperative contract that the State of Utah is seeking to add more vendors to.
The cost points are divided into three categories. Category 1 awards up to 100 points based on the discount percentage offered on catalog items, with higher discounts receiving more points. Category 2 awards up to 100 points based on the average hourly rate for installation/project completion, with the lowest rate receiving the full 100 points. Category 3 applies to vendors who can only sell products and not provide installation, in which case the discount percentage is weighted more heavily.

Most products will require installation and/or modification within an existing structure or vehicle. The lowest hourly rate will receive 100 points. All others will receive a portion of points depending on the ratio of their average installation cost to the lowest. The offeror with the lowest Proposed Price will receive 100% of the price points.
