About this file
This document is an Attachment B Scope of Work for a Multiple Award Contract issued by the State of Utah for patient and wheelchair lift products and related services. The contract covers seven main categories of products and services, including vehicle lifts, portable home lifts, stair lifts, ceiling track lifts, ramps, shower and bath products, and other community and environmental adaptation and accessibility equipment. The purpose is to provide these products and maintenance services to assist people with accessibility in the home and community. Contractors must maintain a current catalog of the offered items, comply with pricing requirements, ensure proper installation and compliance with building codes, and become an Atypical Medicaid Provider if serving Department of Human Services clients.
The contract also outlines specific payment and billing requirements for goods and services provided to DHS/DSPD clients, including the need for written approval, coordination with DHS/DSPD, and invoicing procedures. The contract is governed by federal regulations related to informed choice for vocational rehabilitation services, which requires the provision of detailed information to applicants and recipients about service options and providers. This contract is part of an existing State Cooperative Contract for Patient Mobility Home and Vehicle Modifications, and the State of Utah is seeking to add more vendors to the contract.
