About this file

This document is an attachment related to a state and local contract opportunity from the State of Utah. The contract is for Client Support for Wheelchairs, Lifts, Ramps, Home Modifications, and Accessories. Vendors are required to complete the Attachment C Cost Sheet, which includes providing information on the categories of products and services they offer, the MSRP discount percentage, and the average hourly rate for installation. Cost points are awarded based on the discount percentage offered and the average hourly installation rate, with a total of 200 cost points available. Vendors that are not licensed to provide installation services will have their cost point value doubled.

The contract opportunity is an addition to an existing State Cooperative Contract for Patient Mobility Home and Vehicle Modifications. Pricing terms, set asides, incumbents, and funding sources are not provided in the document. However, it specifies that the vendor with the lowest proposed price will receive 100% of the cost points, and all other vendors will receive a portion based on the ratio of their price to the lowest price.
