About this file
This document is an Insurance Certification Attachment for the California Department of Aging (CDA) RFP Number RFP-2526-01 for an Aging Resource Directory contract. The attachment outlines the insurance requirements and certification process for proposers seeking to bid on the contract. Proposers must acknowledge and understand the insurance requirements detailed in the sample agreement, and if awarded, provide comprehensive insurance documentation including certificates of insurance, proof of coverage for the State of California, and additional insured endorsements.
The certification requires proposers to demonstrate insurance coverage for various types of policies, with specific requirements for Commercial General Liability insurance, particularly when subcontractors are involved. Proposers must ensure their insurance policies cover the State of California, its officers, employees, and agents, either through a policy endorsement or by showing existing coverage. The document emphasizes that proof of insurance is not required with the initial proposal but will be mandatory for awarded firms. CDA reserves the right to cancel a proposed award if the required documentation is not submitted, highlighting the critical nature of these insurance compliance requirements.

Proof that the proposer has insured the State of California, its officers, employees, and agents. Proposer further understands that this proof shall come in the form of either:

2. A copy of the proposer’s current insurance policy that shows that the policy already insures all parties required to be insured by this Agreement.
Proposer certifies that, should Commercial General Liability insurance be required, and should the proposer use a subcontractor to complete a portion of this Agreement, the proposer shall either:
1. Include the subcontractor as an additional named insured under the proposer’s policy, or
2. Ensure that each subcontractor is insured under their own Commercial General Liability policy.
Proposer acknowledges that they understand that CDA reserves the right, at its sole discretion, to cancel a proposed award to a proposer who does not submit the required documentation described herein.
