About this file
This document is a reference guide for Life Safety Code headings used in SVH (Structured Vulnerability/Health) Survey Reports. It establishes seven primary classification categories for documenting violations related to healthcare facility safety compliance.
The categories are: Building Construction (K-Tags digit 1) covering construction type, height, roof systems, and interior wall construction; Means of Egress (digit 2) for egress-related violations; Smoke Barriers and Sprinklers (digit 3) for protection issues including vertical openings, hazardous areas, cooking facilities, alcohol-based hand rubs, interior finishes, fire alarms, sprinkler systems, fire extinguishers, corridors, and smoke compartments; Existing and New High-Rise Buildings and Other (digit 4) for special provision issues; Building Services (digit 5) for elevators, escalators, laundry chutes, gas and electric systems, HVAC, and related infrastructure; Fire Safety and Operations (digit 7) for operating features including fire drills, evacuation plans, smoking policies, fire door testing, space heaters, and construction activities; and Electrical Systems (digit 9) for NFPA 99 Health Care Facilities Code Requirements covering medical gas, electrical systems, emergency generator systems, and electrical equipment. The document notes that K-Tags serve as internal reference identifiers only and are not included in the actual SVH Survey Reports themselves.
