About this file
Facility-Based Survey Observation Record
This document is an evaluation template for monitoring contract surveyor performance during State Veterans Homes (SVH) surveys. The form captures observation data across multiple performance criteria organized into key functional areas: Arrival Expectations, Exit Expectations, Professional Attire, Infectious Disease Protocol, Professional Behavior, Observations of Surveyor(s), Observations of Care, Resident/Family Individual Interviews, Resident Council sessions, Team Conferences, SVH Daily Debrief Conferences, Serious Concern handling, and Survey Determinations. Each criterion includes specific data definitions and observable behaviors that evaluators should assess.
The scoring system uses a 5-point scale (4=Conforms to contract requirements; 3=Minor nonconformance; 2=Does not conform but can be made to conform; 1=Does not conform and cannot be made to conform; 0=Did not observe). The template accommodates evaluation of up to six team members per survey, with columns for individual notes on each Contract Surveyor. Key evaluation areas assess surveyor professionalism, adherence to infectious disease protocols, proper documentation of arrival and exit procedures, consultative interactions with facility staff, appropriate care observations with resident permission and privacy protections, proper interview techniques, coordination among team members, and accurate identification of quality of care, life safety, administrative, and fiscal concerns based on federal regulation requirements. The form concludes with a total average score calculation mechanism.
