About this file

This is a Past Performance Questionnaire (PPQ) template designed to collect reference feedback from previous clients or agencies regarding a contractor's performance on past contracts. The questionnaire is submitted directly to the Air Force Office of Scientific Research (AFOSR) at AFRL.AFOSR.PIA@us.af.mil within 10 calendar days of receipt and becomes part of official source selection records marked as "For Official Use Only."

The PPQ requests detailed information about the contractor's prior performance across multiple dimensions, including overall program management, stakeholder and relationship management, technical quality and expertise, subcontractor or partner management, and proactiveness in problem resolution. Respondents are asked to provide their contact information, their role and tenure on the subject contract, contract details (contract number, type, competitively awarded status, period of performance, and total award value), and a description of services provided and geographic scope. Ratings are provided on a five-point scale: Unsatisfactory (1), Marginal (2), Satisfactory (3), Very Good (4), and Exceptional (5), with a Not Applicable option. The questionnaire concludes with a subjective assessment question asking whether the respondent would award the contract to this contractor today if given a choice, followed by space for general comments and remarks. This PPQ is specific to AFOSR's FY26 Partnership Intermediary Agreement (PIA) solicitation (FA955026RPIA1) and supports evaluation of offerors' ability to execute integrated programmatic and technical support functions related to technology transfer, transition, and collaborative project orders.

PAST PERFORMANCE QUESTIONNAIRE (PPQ)

The Air Force Office of Scientific Research (AFOSR) is currently in the process of awarding a competitive Partnership Intermediary Agreement (PIA) with entities that facilitate joint projects and accelerate technology transfer between the Air Force Research Lab (AFRL) and commercial market under the authority of §15 USC 3715 - Use of Partnership Intermediaries and §10 USC 4214.

The purpose of the partnership intermediary is to increase the likelihood of success in the conduct of joint activities between AFRL, the U.S. Space Force (USSF), Air Force Central Command (AFCENT), the National Reconnaissance Office (NRO), small business firms, and educational institutions. The intermediary will help identify federal technologies that can be licensed and commercialized; and work to develop and grow environments and capabilities for fostering innovation and collaboration necessary for addressing the basic and applied research challenges of AFOSR, AFRL, USSF, AFCENT, and NRO. The Partnership Intermediary Agreement will enable AFOSR to identify, investigate, and exploit research opportunities that will transform future Department of Defense (DoD) capabilities.

The partnership intermediary is expected to focus PIA activities on eight key areas:

i. Partnership development, collaboration facilitation, and agreement enablement;

ii. Technology transfer and early-stage technology transition awareness, market intelligence, and pathway exploration;

iii. Technology scouting, industry engagement, and ecosystem analysis;

iv. STEM and workforce development ecosystem engagement;

v. Access to non-inherently governmental technical expertise, structured studies, and analytic assessments;

vi. Development and use of data tools, analytic platforms, and collaboration or innovation environments;

vii. Governance support, coordination across Collaborative Project Orders (CPOs), and facilitation of convenings, workshops, or engagement activities; and

viii. Production of factual, non-advocacy communications and knowledge-transfer outputs related to technology transfer and transition (T3) activities.

The partnership intermediary is expected to meet the following seven objectives:

i. Advance technology transfer and early-stage technology transition awareness by increasing visibility, pathway identification, and feasibility understanding of opportunities arising from Air Force Research Laboratory (AFRL) and AFOSR-funded basic research and related Department of the Air Force scientific and innovation activities.

ii. Enable informed consideration of downstream maturation, adoption, and commercialization pathways by facilitating early-stage transition insight, stakeholder connectivity, and ecosystem engagement, without presupposing acquisition, fielding, or operational decisions.

iii. Expand and sustain mission-aligned partnerships and collaboration networks across academia, industry, nonprofit organizations, state or local governments, affiliated entities, government laboratories, and other non-Federal partners to leverage external expertise, infrastructure, and innovation ecosystems.

iv. Support partnership and agreement readiness by enabling the effective use of non-FAR collaboration mechanisms and partnership instruments, consistent with statutory authority and Government determinations.

v. Strengthen STEM, workforce, and talent pipeline outcomes aligned with AFRL and AFOSR scientific priorities and long-term Department needs, contributing to the sustainability of the research and innovation ecosystem.

vi. Improve data-enabled insight and decision support through analytics, studies, landscape assessments, and ecosystem intelligence that inform strategy, investment planning, and technology transfer and transition awareness across AFRL, AFOSR, and aligned mission partners.

vii. Enhance transparency, accountability, and effectiveness in the planning, execution, and assessment of PIA-enabled activities to support stewardship of Government research investments and maximize their potential long-term impact
