About this file
This is a Past Performance Questionnaire used to evaluate contractor performance on previous or ongoing contracts relevant to Solicitation No. FA301026Q0032. The questionnaire is issued by the 81st Contracting Squadron at Keesler Air Force Base, Mississippi, for a project involving extending the perimeter fence at the Division Street gate. Offerors submitting proposals must include references who can provide performance feedback, and those references are required to complete and submit this questionnaire directly to the Contracting Officer, Alicia Bowman, at alicia.bowman@us.af.mil no later than 9:00 a.m. CDT on September 9, 2026. Submissions must originate from a corporate or government email account.
The questionnaire requests information about the offeror and the referenced contract, including the contract number, total contract value including all modifications, period of performance, and a brief description of work required. References must evaluate the contractor across five performance categories: Quality of Work (ability to meet work requirements and overall quality), Schedule (timeliness of end-products and ability to resolve timeliness problems), Management (ability to manage all aspects of work), Regulatory Compliance (ability to meet safety regulations, Davis-Bacon Act, and other labor standards), and an Overall Rating. Each category uses a six-point scale: Exceptional (exceeds many requirements with few minor problems), Very Good (exceeds some requirements with some minor problems), Satisfactory (meets requirements with some minor problems), Marginal (meets some requirements with serious unresolved problems), Unsatisfactory (does not meet requirements with ineffective corrective actions), and Not Applicable (information not recent or relevant). Respondents are required to provide narrative comments supporting their ratings.
