B. TANK INSPECTION COMMENTS

1. Tank Construction

Tank 13 is a welded, steel, vertical, JP-5 aboveground storage tank (AST). The original tank manufacturer is unknown. According to the previous inspection report, the tank was constructed IAW with design standard API 12C in 1951. Tank 13 is 45’-0” in diameter and 30’-0” high with a nominal capacity of 8,500 barrels (BBL) (357,000 gallons). The previous out-of-service (OOS) inspection was performed in June

2014 and the report was available for review and comparison.

2. Tank Foundation

Tank 13 rests on a concrete ring wall foundation positioned at grade level. The tank chime-to-ring wall interface is covered with soil and vegetation at multiple locations, allowing the potential for corrosion at the tank chime. A maintenance program should be implemented to routinely remove all soil and vegetation from the concrete ring wall foundation and tank chime-to foundation interface.

A settlement survey was performed at the tank foundation. No signs of settlement were noted. See

Appendix A for settlement results.

3. Cathodic Protection

The tank is equipped with an ICCP system for protecting the tank bottom from corrosion.

4. Tank Shell

The tank shell is equipped with five (5) butt-welded steel courses. Shell plates were examined using ultrasonic thickness testing (UTT) and visual testing (VT) methods. Nine (9) UTT measurements were taken on each plate of the 1st shell course. Nine (9) UTT measurements were taken on the remaining shell courses using the spiral staircase for access. The lower portion of the 1st shell course is provided with multiple insert plates and lap patches. One (1) UTT reading was taken on each insert plate. The average thickness of the insert plates was 0.251” and values ranged from 0.237”-0.306”. Insert plates are of similar size to the first shell course. UTT measurements indicated no significant metal loss on shell plates.

The tank is provided a spiral staircase for access to the fixed roof. There are sixty-four (64) stair treads at support rods provided for the spiral staircase. All sixty-four (64) stair treads are not fully seal-welded to the tank shell allowing water ingress and crevice corrosion. All sixty-four (64) stair support rods attached at the shell are not provided bearing plates. Bearing plates help distribute the load on the tank shell alleviating stresses at attachment locations.

The tank shell is not provided with an emergency responder diamond, IAW National Fire Protection

Agency (NFPA) 704, to identify hazard ratings for health, flammability, and instability. The ratings aid emergency responders in quickly assessing the severity of the situation and allow a proper response.

Current product designation labeling on the tank shell is fading and peeling. Tank 13 is not provided with a data plate.

Refer to Drawing A-1, “Shell Roll-Out” and Table 1, “Shell Ultrasonic Thickness Readings” in Appendix A for plate location and thickness data.

5. Fixed Roof

The fixed roof is of aluminum geodesic dome construction. The aluminum geodesic dome was examined using VT methods. The inspection was limited to areas adjacent to the spiral staircase upper platform.

Geodesic dome appeared to be in good overall condition with no signs of voids or deterioration in sealant.

Facility personnel stated they do not have issues with the dome leaking.

6. Tank Appurtenances

a. Tank Shell

Shell nozzles consist of one (1) 32” manway, one (1) 20” manway , one (1) 1 ½” water draw-off

(flanged), one (1) 6” low suction (flanged), one (1) 6” main suction, one (1) 6” receipt, one (1) 1 ½” plugged nozzle, four (4) 1” penetrations for the HLA/HHLA/HLCV and LLA, and one (1) penetration for the aqueous film forming foam (AFFF) chamber. Shell nozzles and reinforcement pads were examined using UTT and VT methods. Four (4) UTT measurements were taken on each nozzle, over

2”, with a welded connection, at each of four quadrants (0⁰, 90⁰, 180⁰, 270⁰). One (1) UTT measurement was taken on each nozzle with a welded connection 2” or less. One (1) UTT measurement was taken on each associated reinforcement pad.

Pitting was identified on the product receipt piping with a maximum depth measured to be approximately 0.160”. This depth will exceed the structural thickness limit of 0.113” in 1.2 years, specified in API 574, Table 6. Tell-tale holes at shell nozzle reinforcement pads are unplugged and absent of lithium grease. Filling unplugged tell-tale holes with grease helps prevent water ingress, insect infestation, and build-up of debris. Corrosion staining was observed at tell-tale holes. Main suction and receipt lines are not provided with product flow direction arrows and existing product designation labels are fading, peeling and have become illegible. Forty-two (42) fasteners provided at manway 1 (MW1) and manway 2 (MW2) exhibit signs of deterioration with minor metal loss.

Tank 13 is not provided a water draw-off system or product recovery tank (PRT). The current water-draw line is provided with a ball valve. The PRT should be configured to facilitate wet-testing of the HLA/HHLA float chambers. Wet-testing should be controlled via transfer pump, which will supply and drain fuel to each chamber.

Refer to Refer to Drawing A-1, “Shell Roll-Out” and Table 2, “Nozzles and Appurtenances” in

Appendix A for nozzle location and data.

b. Fixed Roof

The geodesic dome fixed roof consists of one (1) 8” Enraf automatic tank gauge (ATG) and one (1) inspection hatch.

c. Gauging

The tank is provided with an 8” Enraf ATG system for automatic tank gauging, and a Varec 2500B tape level gauge (TLG) for monitoring product level from the ground. The TLG failed the bump test indicating the spring may not be actuating the tape adequately, potentially resulting in inaccurate liquid level readings. The tank is not provided with a Vito temperature/water combination probe.

To house the Vito temperature/water combination probe, one (1) 8” roof flanged nozzle should be installed as close to the perimeter sump as feasibly possible. The tank is not provided a manual gauging hatch.

d. Level Alarms

The tank is provided with independent shell-mounted HLA/HHLA and LLA systems. Coating provided at piping for the level alarms is compromised displaying signs of flaking, deterioration, and base metal exposure with surface corrosion.

e. Stairs and Platforms

The tank is equipped with a spiral staircase for accessing the roof. A through-hole exhibiting signs of active corrosion was identified on the upper platform of the spiral staircase. The structural integrity of the platform may be compromised if the through-hole continues to propagate in size resulting in potential hazards for users.

f. Grounding and Bonding

The tank shell is equipped with three (3) grounding lugs and cables that are in good overall condition with no signs of damage.

7. Tank Coating

The tank coating is in poor overall condition exhibiting signs of widespread failure and surface corrosion. A coating program should be implemented to repair identified areas of compromise.

External coating at the tank shell has deteriorated and exhibits significant chalking, indicating the polyurethane topcoat is near the end of its life cycle. A coating condition assessment of the external shell should be performed prior to recoating to determine the condition of the base coat. Additionally, the external shell coating exhibits flaking, deterioration, corrosion staining, and base metal exposure with surface corrosion.

The Enraf ATG exhibits coating failure with signs of flaking, deterioration, and base metal exposure and corrosion staining. Coating for the spiral staircase exhibits minor instances of failure with base metal exposure and surface corrosion. Forty-two (42) stair rod supports exhibit coating failure with signs of flaking, deterioration, and base metal exposure with surface corrosion. All nozzles and appurtenances attached at the tank shell exhibit severe coating failure with signs of flaking, deterioration, and base metal exposure with surface corrosion.

8. Tank Piping System

Carbon steel flanges provided at piping for the level alarms, receipt, main suction, recirculation (both main suction and receipt), and low point drains are attached directly, without isolation, to stainless steel flanges via stainless steel nuts and bolts. This configuration causes significant deterioration of the carbon steel flanges as a result of galvanic corrosion. The underside of the main suction, receipt and recirculation pipelines are wrapped. Wrapping prevents full examination of piping systems and may cause accelerated corrosion due to water retention between the pipe and wrapping.

Carbon steel piping provided for the AFFF is supported with stainless-steel pipe supports. Currently, the dissimilar metals are not provided proper isolation, via a Teflon pad or equivalent, and are in direct contact. This situation may lead to galvanic corrosion due to varying electrochemical potentials of the metals. Crevice corrosion was identified at the interface between the stainless-steel pipe supports and the AFFF piping. The coating provided for the AFFF piping is in poor overall condition and requires recoating. The AFFF piping should be recoated red IAW with MIL-STD-161H, Section 4.3.3.

Crossover piping provided for the main suction and receipt pipelines are equipped with stainless-steel ball valves. Four (4) stainless-steel ball valves are provided with carbon steel handles exhibiting signs of deterioration due to galvanic corrosion. The low suction line is currently flanged directly after the double block and bleed (DB&B) valve. This section is not adequately supported which results in the entire weight of the piping and associated DB&B valve resting on the tank shell. This configuration may result in structural damage to the tank shell.

9. Fire Protection System

For fire protection, Tank 13 is provided with an AFFF system.

10. Secondary Containment

Tank 13 is located within an earthen containment dike with vertical concrete walls that are lined with a flexible membrane liner (FML). The integrity of the FML could not be verified, as it is currently covered with a layer of soil and vegetation approximately 6” in depth. Penetrations at the containment dike walls for the AFFF piping and electrical conduit are not adequately sealed exhibiting signs of degraded material and compromised sealant respectively.

Integrity of the containment dike drainage system should be verified as the current facility procedure is to wait for any liquid within the dike area to evaporate. This impedes access to the tank and prevents adequate inspection for “sheen”. The presence of sheen signifies petroleum may have released into the containment area creating a potential risk for harmful environmental effects. Additionally, it should be noted that a construction crew working in the adjacent containment dike noted water continually entering the dike drain pit despite efforts to drain liquid out.

11. Out-of-Roundness Survey

An out-of-roundness (OOR) survey was performed on the tank shell to detect any deformations of the shell. Results of the OOR survey yielded both inward and outward deflections. The maximum inward deflection observed was 0.261’ (3.131”). The deflection is located 27.41’ above the shell-to-bottom weld and 41.2° (arc length of 16.18’) from manway 1. The maximum outward deflection observed was 0.221’

(2.647”). The deflection is located 27.41’ above the shell-to-bottom weld and 183.5° (arc length of 72.06’) from manway 1. API 650 Section 7.5.3 provides an OOR radius tolerance standard that is not to exceed ±

¾". It should be noted this requirement is for newly constructed tanks. Analysis consisted of examining tank shell deviations from a best fit cylinder at 2.95” intervals in the y-direction and 1.5’ intervals in the x-direction beginning at the shell-to-bottom weld below manway 1.

It should be noted no visual observation of OOR was observed on the tank shell during the inspection.

The out-of-tolerance inward and outward deflections is likely due to out-of-tilt error caused during the

3D scanning phase. The 3D scanner was placed on a metal platform, extending the length of the containment. Slight shifts in weight and vibrations due to scanning, cause shifts in tilt of the scanner, resulting in a larger error in data collected. Refer to Drawings B-1 through B-3 for OOR mapping.

C. FINDINGS AND RECOMMENDATIONS

The following subsections outline the itemized tank repairs/upgrades. Repairs and upgrades shall be completed

IAW API and AW 78-24-78 requirements.

OBSERVATION: Current ringwall has tank resting on grade level. Vegetation, soil, and water was observed resting on the tank chime.

CODE REFERENCE: UFC 3-460-01 Table 8-8.5.A

RECOMMENDATION: Provide the tank with an elevated ring wall. Current ring wall has tank resting on grade level. At a minimum, implement routine program for removing all soil and vegetation from the tank chime.

r. Valve Handles

OBSERVATION: Carbon steel handles provided at four (4) stainless-steel ball valves on the receipt and main suction crossover piping have deteriorated as a result of galvanic corrosion.

CODE REFERENCE: UFC 3-460-01, Section 3-6.7.2

RECOMMENDATION: Replace four (4) carbon steel ball valve handles with stainless steel handles. Two

(2) ball valve handles are located on the receipt crossover piping, and two (2) ball valve handles are located on the main suction crossover piping.

s. Manual Gauging Hatch

OBSERVATION: Tank is not provided with a manual gauging hatch for gauging operations.

CODE REFERENCE: UFC 3-460-01 Table 8-1.S

RECOMMENDATION: Provide the tank with a manual gauging hatch via one (1) 10” roof flanged nozzle and a fully slotted aluminum stilling well. The stilling well should extend to within 3” of the tank bottom and a datum plate should be installed to establish a gauging zero point.

t. Water Draw-Off System and Product Recovery Tank (PRT)

OBSERVATION: Tank is not provided with a water draw-off system and PRT. Water draw-off line is provided with a ball valve.

CODE REFERENCE: UFC 3-460-01 Section 8-3.14 and AW 78-24-27 Sheet D.13 Detail 5

RECOMMENDATION: Provide a water draw-off system with a PRT. Install a ½” stainless steel piping between the PRT and HLA/HHLA chambers to facilitate wet-testing of each float chamber.

u. Pipe Support

OBSERVATION: The low suction pipeline is not provided support, causing the entire weight of piping and the associated DB&B valve to impact the tank shell.

CODE REFERENCE: AW 78-24-27 Sheet D.02

RECOMMENDATION: Provide one (1) support for the low suction line.

v. Bearing Plates on Tank Shell

OBSERVATION: Support rods are partially welded to shell with signs of corrosion staining at connection points.

CODE REFERENCE: BMP

RECOMMENDATION: Provide sixty-four (64) bearing plates to the sixty-four (64) stair support rods, attached directly to the tank shell.

w. Spiral Staircase Upper Platform

OBSERVATION: A through-hole was identified at the upper platform at the spiral staircase. The through hole exhibits active crevice corrosion.

CODE REFERENCE: AW 78-24-27 Sheet D.11

RECOMMENDATION: Provide a 12” circular patch plate over through-hole located on the spiral staircase upper platform.

3. RECOMMENDED REPAIRS as Part of Regular Maintenance

Recommended repairs represent items that should be addressed in a timely manner in order to prevent future potential risks to system operators, equipment integrity, or the adjacent environment.

a. Containment Drainage

OBSERVATION: Due to the existence of soil and vegetation, liquid within the containment area cannot be evaluated for “sheen”. System functionality cannot be verified as functional as current facility procedure waits for liquid within the containment dike to evaporate. Contractors in the adjacent containment area noted water continuously entered dike drain pit despite efforts to continually drain all liquid out.

CODE REFERENCE: BMP

RECOMMENDATION: Perform a hydrostatic test of tank containment drain pit to verify integrity of drain valve and piping.

b. Varec TLG

OBSERVATION: The Varec TLG failed the bump test, indicating the spring may not be actuating the tape adequately leading to inaccurate liquid level readings.

CODE REFERENCE: UFC 3-460-03 Section 8-9.7.1

RECOMMENDATION: Provide repair and maintenance program for the Varec TLG.

D. INSPECTION INTERVALS

Intervals between tank inspections (both internal and external) should be determined by the tank service history, corrosion rates, and regulatory requirements. UFC 3-460-03 provides guidelines for inspection intervals for military facilities. Listed below are the recommended inspection intervals:

4. Routine In-service Inspection

Facility personnel shall perform a visual inspection of the tank exterior surfaces and associated piping.

Gross defects, leaks, shell distortions, signs of settlement, and corrosion should all be monitored along with the foundation condition (including tank chime-to-foundation interface), coating, and appurtenances.

c. Interval: Monthly

5. External Inspection with Ultrasonic Thickness Inspection

An API certified inspector shall perform this inspection every five (5) years or RCA/4N years (where RCA is the difference between the measured shell thickness and the minimum required thickness and N is the shell corrosion rate) whichever is less. This inspection includes inspection requirements of the routine in-service inspection as well as a visual inspection of the tank grounding system (shunts and cables) for damage. Furthermore, an external UT inspection of the tank shell and roof (if applicable) shall be performed by an API certified inspector

d. Interval: Five (5) years; 30 November 2023

6. Internal Inspection

An API certified inspector shall perform the internal inspection every 20 years or ½ remaining life of the tank bottom, whichever is less. This inspection includes a visual inspection of the tank interior surfaces and associated piping and an evaluation of the tank bottom via NDT methods. Internal inspection date is based on recommended inspection interval from the previous API 653 internal inspection report.

e. Interval: March 2026 (API)

*interval based on previous (post repair) inspection dated August 2016

APPENDIX C: API 653 CHECKLISTS FOR TANK INSPECTION

Tank In-Service Inspection Checklist

Item Completed

C.1.1 Foundation

Measure foundation levelness and bottom elevations (see Annex A for extent of measurements).

X

C.1.1.1 Concrete Ring a Inspect for broken concrete, spalling, and cracks, particularly under backup bars used in welding butt-welded annular rings under the shell.

X b Inspect drain openings in ring, back of water draw basins, and top surface of ring for indications of bottom leakage.

X c Inspect for cavities under foundation and vegetation against bottom of tank. X d Check that runoff rainwater from the shell drains away from tank. X e Check for settlement around perimeter of tank. X

C.1.1.2 Asphalt a Check for settling of tank into asphalt base which would direct runoff rain water under the tank instead of away from it.

N/A b Look for areas where leaching of oil has left rock filler exposed, which indicates hydrocarbon leakage.

N/A

C.1.1.3 Oiled Dirt or Sand

Check for settlement into the base which would direct runoff rain water under the tank rather than away from it.

C.1.1.4 Rock

Presence of crushed rock under the steel bottom usually results in severe underside corrosion. Make a note to do additional bottom plate examination (ultrasonic, hammer testing, or turning of coupons) when the tank is out of service.

N/A

C.1.1.5 Site Drainage a Check site for drainage away from the tank and associated piping and manifolds.

X b Check operating condition of the dike drains. X

C.1.1.6 Housekeeping

Inspect the area for buildup of trash, vegetation, and other inflammables buildup.

C.1.1.7 Cathodic Protection

Completed

X

Review cathodic protection potential readings. N/A

C.1.2 Shells

C.1.2.1 External Visual Inspection a Visually inspect for paint failures, pitting, and corrosion. X b Clean off the bottom angle area and inspect for corrosion and thinning on plate and weld.

X c Inspect the bottom-to-foundation seal, if any. X

C.1.2.2 Internal (Floating Roof Tank)

Visually inspect for grooving, corrosion, pitting, and coating failures. N/A

C.1.2.3 Riveted Shell Inspection a Inspect external surface for rivet and seam leaks. N/A b Locate leaks by sketch or photo (location will be lost when shell is abrasive cleaned for painting).

N/A c Inspect rivets for corrosion loss and wear. N/A d Inspect vertical seams to see if they have been full fillet lap-welded to increase joint efficiency.

N/A e If no record exists of vertical riveted seams, dimension and sketch (or photograph) the rivet pattern: number of rows, rivet size, pitch length, and note whether the joint is butt-riveted or lap-riveted.

N/A

C.1.2.4 Wind Girder (Floating Roof Tanks) a Inspect wind girder and handrail for corrosion damage (paint failure, pitting, corrosion product buildup), especially where it occurs at tack-welded junction, and for broken welds.

N/A b Check support welds to shell for pitting, especially on shell plates. N/A c Note whether supports have reinforcing pads welded to shell. N/A

C.1.3 Shell Appurtenances

C.1.3.1 Manways and Nozzles a Inspect for cracks or signs of leakage on weld joint at nozzles, manways, and reinforcing plates.

X b Inspect for shell plate dimpling around nozzles, caused by excessive pipe deflection.

X c Inspect for flange leaks and leaks around bolting. X d Inspect sealing of insulation around manways and nozzles. N/A e Check for inadequate manway flange and cover thickness on mixer manways.

N/A

C.1.3.2 Tank Piping Manifolds a Inspect manifold piping, flanges, and valves for leaks. X b Inspect firefighting system components. X c X

Completed

X Check for anchored piping which would be hazardous to the tank shell or bottom connections during earth movement.

d Check for adequate thermal pressure relief of piping to the tank. X e Check operation of regulators for tanks with purge gas systems. N/A f Check sample connections for leaks and for proper valve operation. N/A g Check for damage and test the accuracy of temperature indicators. N/A h Check welds on shell-mounted davit clips above valves 6 in. and larger. N/A

C.1.3.3 Autogauge System a Inspect autogauge tape guide and lower sheave housing (floating swings) for leaks.

X b Inspect autogauge head for damage. X c Bump the checker on autogauge head for proper movement of tape. X d Identify size and construction material of autogauge tape guide (floating roof tanks).

N/A e Ask operator if tape tends to hang up during tank roof movement (floating roof tanks).

X f Compare actual product level to the reading on the autogauge (maximum variation is 2 in.).

X g On floating roof tanks, when the roof is in the lowest position, check that no more than two feet of tape are exposed at the end of the tape guide.

N/A h Inspect condition of board and legibility of board-type autogauges. N/A i Test freedom of movement of marker and float. N/A

C.1.3.4 Shell-Mounted Sample Station a Inspect sample lines for function of valves and plugging of lines, including drain or return-to-tank line.

N/A b Check circulation pump for…
