B. TANK INSPECTION COMMENTS
1. Tank Construction
Tank 16 is a welded, steel, single-wall, Diesel (F-76), vertical, aboveground storage tank (AST). The original tank manufacturer is unknown. According to the previous inspection report, the tank was constructed IAW with design standard API 12C in 1954. Tank 16 is 45’-0” in diameter and 48’-0” high with a nominal capacity of 13,000 BBL (567,000 gallons). The previous Out-of-Service (OOS) inspection report, dated September 2014, and post-repair inspection report, dated September 2016, were available for review and comparison.
2. Tank Foundation
Tank 16 rests on an elevated concrete ring wall foundation. Foundation is in good overall condition with no indications of spalling or cracking. A settlement evaluation was performed during the inspection. No signs of settlement were noted. See Appendix A for settlement results.
3. Cathodic Protection
The tank is equipped with an ICCP system for protecting the tank bottom from corrosion.
4. Tank Shell
The tank shell is equipped with seven (7) butt-welded steel shell courses. Shell plates were examined using ultrasonic thickness testing (UTT) and visual testing (VT) methods. Nine (9) UTT measurements were taken on each plate of the 1st shell course. Nine (9) UTT measurements were taken on the remaining shell courses, using the spiral staircase for access. UTT measurements indicated no significant metal loss on shell plates.
The tank shell is not provided with an National Fire Protection Agency (NFPA) emergency responder diamond to identify hazard ratings for health, flammability, and instability. The ratings aid emergency responders in quickly assessing the severity of the situation and to allow a proper response. Grounding conduit provided for the tank is open to the environment, allowing fluids to escape the containment in the event of a spill. Tank 16 is not provided with a data plate.
Two (2) locations of general pitting on the exterior of the 1st shell course, adjacent to manway 1, were identified. Pitting was observed at a height of 2’-0” and 5’-0”, with max pit depths of 0.080” and 0.110”, respectively. Pitting is not considered active as it is coated. Pitting does not exceed minimum thickness criteria to affect safe liquid level. These locations should be monitored in future inspections.
Refer to Drawing A-1, “Shell Roll-Out”, and Table 1, “Shell Ultrasonic Thickness Readings”, in Appendix A for plate location and thickness data.
5. Fixed Roof
The fixed roof is of cone-up construction with fully coated, lap-welded steel plates. Fixed roof plates were examined using UTT and VT methods. Five (5) UTT measurements were taken on each fixed roof plate.
UTT measurements indicated no significant metal loss on fixed roof plates.
The fixed roof is provided with a partial handrail. Without adequate fall protection, there is no means to safely access the Enraf ATG or other appurtenances. Seven (7) conduit supports located on the fixed roof are stitch welded exhibits coating failure and crevice corrosion. The tank roof is not provided with an 8” flanged nozzle equipped with a Vito temperature/water probe. The Vito temperature/water probe should be located as close to the sump as possible.
Wide-spread pitting was observed on the fixed roof. At the time of the inspection, none of the pitting exceeds the API 653 requirement that no 100 in2 area shall have an average thickness of less than 0.090”.
Furthermore, all pitting was coated and can be considered inactive. These locations should be monitored throughout future inspections.
Refer to Drawing A-2, “Fixed Roof Layout”, and Table 2, “Fixed Roof Ultrasonic Thickness Readings”, in
Appendix A for plate location and thickness data.
6. Tank Appurtenances
a. Tank Shell
Shell nozzles consist of twelve nozzle penetrations: two (2) 20” manways, one (1) 1” plug, one (1) 2” low suction, one (1) 6” main suction, one (1) 6” receipt, one (1) 1” HLA /HLCV float pilot, one (1) HHLA / HLCV float pilot, two (2) 1” LLA penetrations, one (1) aqueous film forming foam (AFFF) penetration, and one (1) 6” blind flange. Shell nozzles and reinforcement pads were examined using UTT and VT methods. One (1) UTT measurement was taken on each nozzle with a welded connection less than 2”, and four (4) UTT measurements were taken on each nozzle with a welded connection over 2”. One (1) UTT measurement was taken on each associated reinforcement pad. UTT measurements indicated no significant metal loss on nozzles and associated reinforcement pads.
Hardware provided for manway 1 and manway 2 exhibit coating failure and metal loss.
Crossover piping provided for the receipt and main suction are provided stainless steel piping and flanges. These stainless-steel flanges are attached directly to carbon steel flanges of the carbon steel piping. The HLA / HLCV, HHLA / HLCV, and LLA are provided similar configurations. This configuration causes significant deterioration of carbon steel components due to galvanic corrosion.
Refer to Refer to Drawing A-1, “Shell Roll-Out”, and Table 3, “Nozzles and Appurtenances”, in
Appendix A for nozzle location and data.
b. Fixed Roof
Fixed roof nozzles consist of eight (8) nozzle penetrations: one (1) 24” manway with 4” Enraf
ATG, two (2) 2” penetrations for the Varec 2500B tape level gauge (TLG) guide wires, one (1)
2 ½” penetration for the Varec TLG, one (1) 4” anchor mount, one (1) 24” inspection hatch, one (1) 8” manual gauging hatch, and one (1) 6” perimeter vent. Fixed roof nozzles and reinforcement pads were examined using UTT and VT methods. Four (4) UTT measurements were taken on each nozzle with a welded connection over 4”, and one (1) UTT measurement was taken on each associated reinforcement pad. UTT measurements indicated no significant metal loss on nozzles and associated reinforcement pads.
Refer to Refer to Drawing A-2, “Fixed Roof Layout,” and Table 3, “Nozzles and
Appurtenances,” in Appendix A for nozzle location and data.
c. Gauging
The tank is provided with an 8” manual gauging hatch for manual operations, a 4” Enraf ATG system mounted on manway 3 for automatic tank gauging, and a Varec 2500B TLG for monitoring product level from the ground. Inspection of stilling wells was not possible due to inaccessibility. The Varec TLG failed the bump check at the time of the indication, indicating the spring is not actuating the tape, potentially causing an inaccurate level reading.
d. Level Alarms
The tank is provided with independent shell-mounted HLA/HHLA and LLA systems.
e. Stairs and Platforms
The tank is provided with a spiral staircase and one (1) upper platform for fixed roof access.
Sixty-four (64) stair treads are stitch welded to the shell, allowing for crevice corrosion.
Coating failure and crevice corrosion were observed at the stair treads. The spiral staircase does not have a concrete pad beneath the first step, creating a safety hazard for personnel accessing the fixed roof, HLA, HHLA, HLCV float pilot, and the AFFF.
f. Grounding and Bonding
The tank shell is equipped with two (2) grounding lugs and cables. Grounding cables are in good overall condition with no signs of damage nor deterioration.
7. Tank Coating
Tank 16 is provided with a two (2) coat epoxy one (1) topcoat polyurethane coating system applied by
Purcell Painting in June 2016. Original average coating thickness ranges from 8-13 mils. MIL-DTL-2441
Formula 159 was used as the primer, and MIL-DTL-2441 Formula 152 was used as the intermediate.
Tank surface was prepared IAW SSPC SP-10 (NACE 2) Near-White Blast Cleaning with an anchor profile of 3 mils. Tank shell coating is in good overall condition. Fixed roof coating is in good overall condition with minor areas of flaking, deterioration, base metal exposure, and surface corrosion.
Coating failure was observed on the spiral staircase, HLA and flange, HHLA and flange, LLA, manway 1 and 2, AFFF piping, low point drains on main suction and receipt piping, and receipt nozzle. Coating displays flaking, deterioration, base metal exposure, and surface corrosion. Coating provided for manway
3 and the 24” inspection hatch and associated hardware displays flaking, deterioration, base metal exposure, and surface corrosion.
8. Tank Piping System
The tank is provided with one (1) 2” low suction, one (1) 6” main suction, one (1) 6” receipt, and one (1)
4” AFFF. The 6” main suction, located at the containment wall penetration, exhibits coating failure and minor metal loss. AFFF piping system is not provided with isolation between pipe supports and piping.
Metal-to-metal contact between dissimilar metals accelerates contact corrosion. Coating failure and crevice corrosion were observed. The AFFF pipe penetrates the ground inside containment. The soil-to-air interface was not accessible for inspection. Acidic and low resistive soils may accelerate corrosion at locations of coating failure.
9. Fire Protection System
For internal fire protection, Tank 16 is provided with an AFFF system.
10. Secondary Containment
Tank 16 is located within an earthen containment dike lined with a flexible membrane liner (FML) and provided with vertical concrete walls. The integrity of the FML could not be verified, as it is currently covered with a 6” layer of soil and vegetation resulting in limited visibility of the FML. According to facility personnel, containment drain pump and piping is not used. Facility allows water to naturally evaporate, therefore, integrity of the containment drain, pump, and piping was unable to be verified due to standing water. Furthermore, this system impedes access to the tank and prevents adequate inspection for sheen.
Conduit inside the containment, adjacent to the 6” receipt double block and bleed (DB&B) valve, is severed and is no longer sealed. One (1) junction box is not sealed and is open to the atmosphere. In the event of a spill, fluid can escape through the junction box support hole on the east side of the containment.
11. Out-of-Roundness Survey
An out-of-roundness (OOR) survey was performed on the tank shell to detect any deformations of the shell. Results of the OOR survey yielded both inward and outward deflections. The maximum inward deflection observed was 0.077’ (0.923”). The deflection is located 1.02’ above the shell-to-bottom weld and 250.8° (arc length of 98.62’) from manway 1. The maximum outward deflection observed was 0.104’
(1.250”). The deflection is located 1.02’ above the shell-to-bottom weld and 363.2° (arc length of 142.8’) from manway 1. API 650 Section 7.5.3 provides an OOR radius tolerance standard that is not to exceed ±
¾". It should be noted this requirement is for newly constructed tanks. Analysis consisted of examining tank shell deviations from a best fit cylinder at 2.95” intervals in the y-direction and 1.5’ intervals in the x-direction beginning at the shell-to-bottom weld below manway 1. Refer to Drawings B-1 through B-3 for OOR mapping.

HL = The height above the bottom of the course of study to the maximum liquid level height of the product, in feet. For corroded or pitted areas, HL = the height from the bottom of the corroded or pitted area to the maximum liquid level height of the product, in feet.
Hc = Height of shell course, in feet.
tmin = The calculated minimum acceptable shell thickness, in inches (cannot be less than 0 for any course), or the minimum acceptable shell thickness allowed by API-653 for tank size, in inches, whichever is greater.
D = Nominal diameter of tank, in feet.
G = Highest specific gravity of the contents (including test water if tank will, or may, be tested in the future).
S = Maximum allowable stress, in pounds per square inch. For welded tanks; use the smaller of 0.80Y of 0.429T for bottom and second course or the smaller of 0.88Y or 0.472T for all other courses. For riveted tanks; S = 21,000 psi.
Y = Specified minimum yield strength of the plate, in pounds per square inch; use 30,000 psi if not known (N/A for riveted tanks).
T = The smaller of the specified minimum tensile strength of the plate or 80,000 psi; use 55,000 psi if not known (N/A for riveted tanks).
E = Original joint efficiency for the tank. For welded tanks; use API-653, Table 2-1.
Note: Use E = 1.0 when evaluating the retirement thickness in a corroded plate, when away from welds or joints by at least the greater of one inch or twice the plate thickness. For riveted tanks; use E = 1.0 for shell plates greater than 1 inch away from rivets; use the value of E from API-653 Table 2-1 when within 1 inch of rivets.

APPENDIX C: API 653 CHECKLISTS FOR TANK INSPECTIONS
Tank In-Service Inspection Checklist
Item Completed
C.1.1 Foundation
Measure foundation levelness and bottom elevations (see Annex A for extent of measurements).
X
C.1.1.1 Concrete Ring a Inspect for broken concrete, spalling, and cracks, particularly under backup bars used in welding butt-welded annular rings under the shell.
X b Inspect drain openings in ring, back of water draw basins, and top surface of ring for indications of bottom leakage.
X c Inspect for cavities under foundation and vegetation against bottom of tank. X d Check that runoff rainwater from the shell drains away from tank. X e Check for settlement around perimeter of tank. X
C.1.1.2 Asphalt a Check for settling of tank into asphalt base which would direct runoff rain water under the tank instead of away from it.
N/A b Look for areas where leaching of oil has left rock filler exposed, which indicates hydrocarbon leakage.
N/A
C.1.1.3 Oiled Dirt or Sand
Check for settlement into the base which would direct runoff rain water under the tank rather than away from it.
C.1.1.4 Rock
Presence of crushed rock under the steel bottom usually results in severe underside corrosion. Make a note to do additional bottom plate examination (ultrasonic, hammer testing, or turning of coupons) when the tank is out of service.
N/A
C.1.1.5 Site Drainage a Check site for drainage away from the tank and associated piping and manifolds.
X b Check operating condition of the dike drains. X
C.1.1.6 Housekeeping
Completed
Inspect the area for buildup of trash, vegetation, and other inflammables buildup.
X
C.1.1.7 Cathodic Protection
Review cathodic protection potential readings. N/A
C.1.2 Shells
C.1.2.1 External Visual Inspection a Visually inspect for paint failures, pitting, and corrosion. X b Clean off the bottom angle area and inspect for corrosion and thinning on plate and weld.
X c Inspect the bottom-to-foundation seal, if any. X
C.1.2.2 Internal (Floating Roof Tank)
Visually inspect for grooving, corrosion, pitting, and coating failures. N/A
C.1.2.3 Riveted Shell Inspection a Inspect external surface for rivet and seam leaks. N/A b Locate leaks by sketch or photo (location will be lost when shell is abrasive cleaned for painting).
N/A c Inspect rivets for corrosion loss and wear. N/A d Inspect vertical seams to see if they have been full fillet lap-welded to increase joint efficiency.
N/A e If no record exists of vertical riveted seams, dimension and sketch (or photograph) the rivet pattern: number of rows, rivet size, pitch length, and note whether the joint is butt-riveted or lap-riveted.
N/A
C.1.2.4 Wind Girder (Floating Roof Tanks) a Inspect wind girder and handrail for corrosion damage (paint failure, pitting, corrosion product buildup), especially where it occurs at tack-welded junction, and for broken welds.
N/A b Check support welds to shell for pitting, especially on shell plates. N/A c Note whether supports have reinforcing pads welded to shell. N/A
C.1.3 Shell Appurtenances
C.1.3.1 Manways and Nozzles a Inspect for cracks or signs of leakage on weld joint at nozzles, manways, and reinforcing plates.
X b Inspect for shell plate dimpling around nozzles, caused by excessive pipe deflection.
X c Inspect for flange leaks and leaks around bolting. X d Inspect sealing of insulation around manways and nozzles. N/A e Check for inadequate manway flange and cover thickness on mixer manways.
N/A
C.1.3.2 Tank Piping Manifolds a Inspect manifold piping, flanges, and valves for leaks. X
Completed
X b Inspect firefighting system components. X c Check for anchored piping which would be hazardous to the tank shell or bottom connections during earth movement.
X d Check for adequate thermal pressure relief of piping to the tank. X e Check operation of regulators for tanks with purge gas systems. N/A f Check sample connections for leaks and for proper valve operation. N/A g Check for damage and test the accuracy of temperature indicators. N/A h Check welds on shell-mounted davit clips above valves 6 in. and larger. N/A
C.1.3.3 Autogauge System a Inspect autogauge tape guide and lower sheave housing (floating swings) for leaks.
X b Inspect autogauge head for damage. X c Bump the checker on autogauge head for proper movement of tape. X d Identify size and construction material of autogauge tape guide (floating roof tanks).
N/A e Ask operator if tape tends to hang up during tank roof movement (floating roof tanks).
X f Compare actual product level to the reading on the autogauge (maximum variation is 2 in.).
X g On floating roof tanks, when the roof is in the lowest position, check that no more than two feet of tape are exposed at the end of the tape guide.
N/A h Inspect condition of board and legibility of board-type autogauges. N/A i Test freedom of movement of marker and float. N/A
C.1.3.4 Shell-Mounted Sample Station a Inspect sample lines for function of valves and plugging of lines, including drain or return-to-tank line.
N/A b Check circulation pump for leaks and operating problems. N/A c Test bracing and supports for sample lines and equipment. N/A
C.1.3.5 Heater (Shell Manway Mounted)
Inspect condensate drain for presence of oil indicating leakage. N/A
C.1.3.6 Mixer a Inspect for proper mounting flange and support.
