This Statement of Work is for the NASA Safety & Mission Assurance, Audits, Assessments, and Analysis (SA3) Services contract. This contract is in direct support of the NASA Safety Center (NSC).

1.2 General Description/Scope

The contractor shall provide Safety & Mission Assurance, Audits, Assessments and Analysis (SA3) Services for auditing of NASA programs and projects.

2.0 Applicable Documents

The following document serves as requirements base for the Contractor’s performance under this SOW:

• NPR 8705.6, Safety and Mission Assurance (SMA) Audits, Reviews, and

Assessments

In order to support an IFOSA audit the contractor shall apply the NASA Requirement Documents corresponding to Electrical Safety, Facility System Safety, Lifting Devices and Equipment, Fire Protection, Explosives Safety, Range Flight Safety, Pressure Vessels and Pressurized Systems, and Mishap and Close Call Reporting.

In order to conduct a QAAR audit the contractor shall apply the NASA requirement documents corresponding to Quality and Software Assurance.

Standards and Requirements Various NASA Procedure and Requirements applicable located at:

http://nodis3.gsfc.nasa.gov/ Various applicable NASA Technical Standards located at:

3.0 Government Furnished Equipment and Services

If the employee needs NASA Information Technology (IT) access, then the government will furnish employees with NEST M-Seats (Windows Lightweight Laptop). This seat comprises a laptop, docking station, mouse, keyboard, and monitor. No other peripheral equipment will be provided. Other equipment including video capture, live streaming and attachments may be furnished temporarily for use on virtual/remote audits and/or assessments.

4.0 Deliverables

Provide a technical approach, schedule, staffing plan including skill mix, cost, and issues/risks for all activities listed in 5.0 General Audit Support Tasks, including Pre- Audit activities, Audit Performance Period activities and Post-Audit activities for each of the Sample Audits (IFOSA and QAAR).

5.0 Requirements

Background The NASA Safety Center (NSC) Assessments and Investigations Office (AIO) conducts Institutional, Facility, Operational Safety Audits (IFOSAs) and Quality Audits, Assessments and Reviews (QAARs) at all NASA Centers, component facilities and the Jet Propulsion Lab (JPL). NSC AIO also performs QAARs of NASA Programs and Projects.

These audits are conducted to verify compliance with NASA policy directives and procedural requirements, agency Safety and Mission Assurance (SMA) requirements, contractual requirements, as well as federal, state, and local safety and health regulations. IFOSAs are conducted at NASA centers and component facilities at least once every four years, with Process Assessments and ICAs scheduled as necessary in between audits. QAAR schedule and scope are determined by Agency quality risk and program/project/center performance. Audits and Assessments may be conducted remotely via desktop review with or without video capture and live streaming equipment, on-site or a combination of both methods.

Institutional, Facility, Operational Safety Audit (IFOSA) The objective of the IFOSA is to ensure a safe work environment by verifying that NASA centers — including the Jet Propulsion Laboratory and component facilities — as well as NASA programs and projects, are in compliance with all applicable facility and operational safety requirements. Using verifiable objective evidence, the IFOSA provides a baseline for understanding the performance of an auditee’s safety program. It provides an independent and constructive evaluation of the effectiveness, efficiency, implementation and adherence of safety requirements for all levels of management.

The functional areas included in the scope of the IFOSA are:

• Electrical Safety

• Explosives Safety

• Facility System Safety

• Fire Protection

• Lifting Devices and Equipment

• Mishap and Close Call Reporting, Investigating and Recordkeeping

• Pressure Vessels and Systems

• Range Safety, where applicable

Quality Audit, Assessment and Review (QAAR) The objective of the QAAR is to provide independent verification that NASA centers, programs and projects are in compliance with applicable agency Safety and Mission Assurance (SMA) Quality Assurance and Software Assurance requirements.

The scope of the QAAR encompasses the agency’s SMA Quality Assurance and Software Assurance requirements with an emphasis on NASA centers, program and project prime contractors, and center support service contractors.

The audited areas included, but not limited to, in the scope of the QAAR are:

• Materials Control

• Metrology and Calibration

• Process Performance Evaluation and Improvement

• Purchasing/Contracts

• Software Assurance

• Work Acceptance

• Work Controls

General Audit Support Tasks This task is in direct support of the NASA Safety Center (NSC) and is to provide general audit support for an IFOSA and QAAR. The audit support includes administrating, planning, scheduling, conducting and documenting various and numerous audit tasks. Official audit findings are stored in the System for Tracking Audits/Assessments and Reviews (STAR).

Preparing, assisting, reviewing, documenting and executing the following key items.

o Pre-Audit Activities include:

Review Audit checklist Generate Audit agenda

Provide recommendations report to the Audit Program Manager that details prior audit findings and corrective actions along with special items of concern

Support Pre-Audit Team Meetings (assume two to eight meeting, telecom ~ 1 hour each in length) o Audit Activities include:

Support Audit In-Brief and Out-Brief Presentations Conduct interviews and meetings Review documentation and processes Track objective evidence Draft Final Audit findings Enter audit findings into STAR database o Post Audit Activities include:

Completion of Audit Checklists and Lessons Learned Support post discussion with/to OSMA Final Draft Audit Report Data Collection for Analysis and Document Cataloging Corrective Action Plan (CAP) Closure status reporting Provide Center CAP technical review and feedback (completed in

STAR)

Sample Audits

1. Institutional, Facility, Operational Safety Audit (IFOSA) Kennedy Space Center (KSC) is located at KSC, Florida and is scheduled to have an IFOSA audit conducted April 18 – 22, 2022. The IFOSA scope will focus on three functional areas: Electrical Safety, Explosives Safety, and Pressure Vessels and Pressurized Systems.

Your task is to provide the audit support to conduct an IFOSA audit “on site” at KSC.

Access to KSC and all necessary procedures and documentation will be provided to the auditors.

Provide your technical approach, schedule, staffing plan including skill mix, cost, and issues/risks for all activities listed in General Audit Support Tasks, including Pre-Audit activities, Audit Performance Period activities and Post-Audit activities.

2. Quality Audit, Assessment and Review (QAAR) NASA’s New Rocket Program, managed out of Marshall Space Flight Center (MSFC), with support from Glenn Research Center (GRC) and Kennedy Space Center (KSC) is a NASA Class A, Type 1 Program. Prime contractor is XYZoom Technical (XYZ) was awarded the contract and the program has completed Preliminary Design Review (PDR). NSC AIO will perform a “virtual/remote” Quality Audit, Assessment and Review (QAAR) of the program. The QAAR scope will focus on Materials Control, Software Assurance and Purchasing/Contracts. All procedures and documentation will be provided to the auditors. The Project is available for interviews beginning July 11, 2022. The Audit Outbrief will be conducted on July 29, 2022.
