About this file
This is a credit release form for the Court Services and Offender Supervision Agency (CSOSA). The document serves as a template or procedural document that outlines the process and requirements for releasing credit information related to individuals under CSOSA supervision. The form is designed to be completed by contractors providing supportive housing services as part of the broader solicitation for Supportive Housing Services (Up to 180 Days), solicitation number 9594CS26Q0075.
The credit release form establishes the authorization and documentation needed for CSOSA or its contractors to obtain and review credit history information for individuals being placed in supportive housing programs. This documentation is likely required to assess housing eligibility, verify financial responsibility, or establish baseline information for individuals participating in the supportive housing initiative. As an attachment to the main solicitation, this form represents a mandatory compliance and administrative requirement that contractors must implement and utilize when processing placements or admissions into the supportive housing program. Contractors responding to the solicitation should be prepared to incorporate this credit release process into their intake and enrollment procedures.
