About this file
This is a Past Performance Information Disclosure Consent Form template for federal contract solicitation SPE603-26-R-0522 issued by Defense Logistics Agency (DLA) Energy for fuel facility operations and maintenance in Davao, Philippines.
The form serves as a consent document that joint venture partners, major subcontractors, and teaming partners must complete and submit to authorize the Government to disclose their present and past performance information to the prime contractor during source selection discussions. The document explains that because a prime contractor is considered a private party, Government consent from teaming partners is required before releasing their past performance data. Prospective offerors must obtain completed and signed consent forms from all identified joint venture partners, subcontractors, and teaming partners and include them with their Past Performance Information submission. The form requires the signatory to be an individual with authority to legally bind the company, along with the company name and address. The sample demonstrates the appropriate format and language for the consent letter, referencing the Government's emphasis on past performance evaluation to obtain best value in source selection.

Attachment IV – Past Performance Information Consent Form
Davao, Philippines
PAST PERFORMANCE INFORMATION DISCLOSURE CONSENT FORM
Joint Venture Partner/Subcontractor/Teaming Partner Consent Form for the Release of Present and Past Performance Information to the Prime Contractor.
Present and Past Performance Information concerning joint venture partners cannot be disclosed to a private party without the teaming joint venture partner’s consent. Because a prime contractor is a private party, the Government will need that consent before disclosing joint venture/teaming partner’s Past Performance Information to the prime during discussions. DLA Energy requests that the following consent form be completed by the joint venture partners/major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Information.
