About this file
This is a Conflict of Interest Affidavit form for the State of Maryland, specifically required for the Trimble GPS and Laser Range Finder equipment procurement by the Maryland Department of the Environment (MDE). The affidavit, effective July 2024, requires all offerors, contractors, consultants, subcontractors, and their employees or agents with authority over contract work to disclose any actual or potential conflicts of interest that could impair their objectivity or provide unfair competitive advantage. Offerors must warrant that except as disclosed in the affidavit, no relevant facts or circumstances exist that would create a conflict of interest, and they must immediately notify the procurement officer in writing if any actual or potential conflict arises after submission, including a detailed description of actions taken to avoid, mitigate, or neutralize such conflicts.
The affidavit requires authorized representatives to execute the document under penalties of perjury, affirming that all contents are true and correct to the best of their knowledge, information, and belief. The completed affidavit must be submitted with the bid or proposal for the Trimble equipment procurement. This document serves as a compliance mechanism under COMAR 21.05.08.08 and is a standard requirement for state procurement activities, ensuring that all parties involved in contract performance are free from conflicts that could compromise the integrity of the procurement process or the state's ability to receive impartial assistance and advice.
