About this file

This document is an Attachment G Responsibility Inquiry form that is part of a state and local government Request for Proposal (RFP) for a Network Detection and Response (NDR) Solution. The State of Oregon, through the Department of Administrative Services, Procurement Services (DAS PS), is seeking to procure an NDR solution, including all required equipment, configurations, licenses, portals, training, integrations with current security systems, maintenance and operations support. The State prefers a SaaS, IaaS or COTS solution and anticipates awarding one contract from this RFP.

The Responsibility Inquiry form requires the proposer to provide information about their financial, material, equipment, facility and personnel resources, as well as details on their previous contract performance, any criminal or civil charges, outstanding judgments, financial distress, licenses, and insurance/registrations. The proposer must also submit a current Pay Equity Certificate if they employ 50 or more full-time workers and the contract price is estimated to exceed $500,000.
